非诺贝特和苯溴马隆治疗高尿酸血症的效果

摘　　要：目的比较非诺贝特和苯溴马隆联合治疗高尿酸血症的效果及其安全性。方法将45例B超检查泌尿系统无尿酸结石、伴或不伴有高三酰甘油血症的高尿酸血症病人,按不平衡指数最小的原则随机分为甲、乙、丙3组(各15例),甲组给予非诺贝特200mg,乙组给予苯溴马隆50mg,丙组为两种药物联合,均每日1次,治疗10d。各组均同时加用碳酸氢钠1.0g,每日3次碱化体液,均要求低脂低嘌呤饮食。治疗前后检测血尿酸(UA)、三酰甘油、24h尿UA及肝肾功能,治疗结束后行泌尿系统B超检查。结果 3组治疗后血UA水平均较治疗前下降,差异有显著性(t=1.86～2.75,P<0.05);治疗后乙组血UA水平低于甲组,丙组低于甲、乙两组,差异均有显著性(F=3.42,q=5.85～6.98,P<0.01)。3组治疗后24h尿尿酸定量高于治疗前,差异有显著性(t=1.96～2.89,P<0.05);治疗后3组间24h尿尿酸定量比较,差异有显著性(F=3.14,q=5.97～7.16,P<0.05)。3组均未出现肝肾功能异常,未出现关节炎及尿酸结石。结论非诺贝特降尿酸效果略低于苯溴马隆,二者联用降尿酸效果明显高于各药单用。Objective To assess the effects and safety of combination therapy with fenofibrate and benzbromarone in hyperuricemia. Methods Forty five patients with hyperuricemia without sonographic uric acid calculus in urinary system, accompanying or un-accompanying hypertriglyceridemia, were evenly randomized to fenofibrate 200 mg （group A）; benzbromarone 50 mg q.d. （group B） ; and fenofibrate plus benzbromarone （group C）, lasting for 10 days. Sodium bicarbonate, 1.0 g, was added to all patients in the three groups, and low purine diet required. Serum uric acid, triglyceride,uric acid in 24-hour urine, liver and renal functions were detected before and after therapy, and an ultrasonography was done upon completion of the therapy. Results After treatment, serum uric acid levels in the three groups were lower than that before treatment, the differences being significant （t=1. 86-2.75,P〈0.05） ; and the serum uric acid in group B was lower than that in group A;that in group C was lower than that in both groups A and B, all showed statistical significance （F=3.42,q=5.85-6.98,P〈0.01）. The quantitation of uric acid in urine of 24 hours, after treatment, was higher than that before treatment （t=1. 96-2.89,P（0.05） ; and the differences of that between the three groups were also significant （F=3.14 ,q= 5.97-7. 16, P〈0.05）. No abnormal liver or renal function, and no arthritis or uric acid calculus was found in the three groups. Conclusion The effect of fenofibrate in dropping uric acid is a little bit lower than benzbromarone, but the dual therapy with them is much more effective than that of monotherapy of the drug.
