不稳定型心绞痛病人IMA与sC5b-9变化及意义

摘　　要：目的 探讨不稳定型心绞痛（UA）病人缺血修饰清蛋白（IMA）与可溶性补体激活产物（sC5b-9）水平变化及其与心肌缺血的关系.方法 采用酶联免疫吸附（EILSA）法,检测了52例健康人和96例UA病人血清IMA、sC5b-9浓度.结果 UA病人血清IMA、sC5b-9浓度明显高于对照组,差异有极显著性（t′=17.28、24.21,P〈0.01）;心绞痛发作时IMA、sC5b-9浓度增高较缓解后更明显（t′=24.56、6.94,P〈0.01）;心绞痛发作时和缓解后,sC5b-9与IMA呈正相关（r=0.813、0.761,P〈0.01）.不同类型的心绞痛发作时和缓解后sC5b-9、IMA浓度差异亦具有显著性,自发性心绞痛病人各指标增高较心肌梗死后和恶化劳力性心绞痛更明显（F=22.74～158.47,q=7.55～28.71,P〈0.01）.结论 急性心肌缺血与IMA和sC5b-9浓度变化有密切关系.Objective To study the serum levels of ischemia modified albumin （IMA） and soluble complement activation component （sC5b-9） in patients with unstable angina （UA）, and explore the relationship between IMA, sC5b-9 and acute myoeardial ischemia. Methods The serum levels of IMA and sC5b-9 were measured with enzyme linked immunosorbent assay （ELISA） in 96 patients with UA and 52 healthy controls. Results The concentrations of IMA and sC5b-9 were significantly higher in UA patients than that in the controls （t＇=17.28,24.21 ;P〈0.01）. The increase of IMA and sCSb-9 at the attack was much significant than that after relief （t＇= 24.56,6.94;P〈0.01）. There was a positive correlation between sC5b-9 and IMA during angina attack and relief （r= 0. 813,0. 761;P〈：0, 01）. The differences of serum levels of IMA and sC5b-9 in different types of angina during attack and relief were significant （F=22.74--158. 47,q= 7.55--28.71, P〈0.01）. The increase of various parameters in patients with spontaneous angina was much obvious than those with post-infarction and effort angina （F= 22. 74- 158.47, q = 7.55- 28.71 ,P〈0.01）. Conclusion Acute myocardial ischemia is closely related to the concentration changes of IMA and sC5b-9.
