摘  要:目的探讨冠心病病人载脂蛋白(Apo)A5基因变异与冠心病及其脂代谢异常的相关性,并分析其在冠心病发病机制中的作用。方法随机选择非亲属山东汉族冠心病及其脂代谢异常病人275例(冠心病组),以年龄和性别相匹配的正常人289例作为对照组,采用聚合酶链反应-限制性内切酶片断长度多态性分析,结合全自动测序技术,检测两组ApoA5 SNP3、SNP4位点基因型,同时检测血脂水平。结果经Logistic回归分析校正,冠心病组SNP3位点TC基因型与对照组比较差异有显著性(χ2=4.08,P<0.05),且为独立影响因素。ApoA5 SNP3与ApoC3-482C/T基因间的连锁不平衡与对照组比较具有一定的差异。冠心病组SNP3 TC基因型三酰甘油水平明显高于其他两种基因型(F=7.56,q=4.69、4.32,P<0.05)。结论 SNP3与冠心病的遗传易感性相关;SNP4通过影响血脂代谢水平增加冠心病的易感性;单倍体优势是冠心病遗传易感性的重要因素之一。Objective To study the correlation of genovariation of apolipoprotein A5 (Apo) with coronary artery disease (CAD) and its abnormal lipid metabolism. Methods The present study consisted of 275 CAD patients with abnormal lipid metabolism, who were selected in random from non-relatives of Han people of Shandong province; and 289 healthy subjects matching with age and gender served as controls. The gene variation of apolipoprotein series including ApoA5 SNP3 and SNP4 were identified by polymerase chain reaction-restriction fragment length polymorphism and DNA sequence, and plasma lipid levels measured as well. Results After Logistic regression analysis, the frequency of ApoA5 SNP3 TC in the patients was significantly higher than that of the control group (X2=4.08,P〈0.05), which was a independent influencing factor. The strong linkage disequilibrium was reflected between ApoA5 SNP3 and ApoC3-482C/T in CAD group. In CAD patients, the levels of triglyceride (TG) were significantly higher in ApoA5 SNP3 TC compared with the other two genotypes (F= 7.56;q= 4. 69,4.32;P〈0.05). Conclusion ApoA5 SNP3 is associated with genetic predisposition of CAD, and SNP4 increases the susceptibility of CAD through effecting lipid metabolism. The haploid dominance is one of important factors of genetic predisposition of CAD.
