阿维A对寻常型银屑病患者血浆内皮素水平的影响

摘　　要：目的探讨阿维A治疗寻常型银屑病的临床疗效和安全性,及其对血浆内皮素水平的影响。方法寻常型银屑病患者48例,给予阿维A20～30mg与餐同服,至无新发皮疹、原皮疹基本消退时减量为10～20mg继服,共治疗12周。根据治疗前后患者的银屑病皮损面积及严重度指数(PASI)评分的改变评价疗效,并用放射免疫法检测其血浆内皮素水平。结果治疗第8周时总有效率91.7%,12周时为95.8%,未发现严重不良反应。治疗前后血浆内皮素水平分别为(62.91±16.32)和(56.94±17.62)pg.mL-1,差异有显著性(P<0.05)。结论阿维A治疗寻常型银屑病有较好疗效,不良反应小。治疗后血浆内皮素水平明显下降,降低内皮素水平可能为其治疗机制之一。Objective To investigate the clinical efficacy and safety of acitretin in treatment of psoriasis vulgaris and its effect of plasma endothelin（ET）.Methods Forty eight patients with psoriasis vulgaris were treated with 20-30 mg acitretin per day with meal.Dosage decreased to 10-20 mg when no more lesions appeared and old lesions almost disappeared.Patients were totally treated for 12 weeks.Efficacy was evaluated by the change of psoriasis area and severity index（PASI）.Radioimmunoassay was used to determine the plasma levels of ET.Results Total effective rates after eight weeks and twelve weeks＇ treatment were 91.7% and 95.8%,respectively,and no severe side effect was found.Plasma endothelin level was（62.91±16.32）pg·mL-1 before the treatment and decreased to（56.94±17.62）pg·mL-1 after treatment（P0.05）,the difference was significant.Conclusion Acitretin is effective and safe for treatment of psoriasis vulgaris,and plasma ET decreases significantly after treatment,which may contribute to the therapeutic mechanism of acitretin.
