摘  要:目的 探讨彩色多普勒对糖尿病下肢动脉病变(LEADDP)的诊断价值及有关LEADDP的危险因素。方法 应用彩色多普勒检测了50例糖尿病病人的双侧下肢动脉,同时与30例正常人对照;应用多因素逐步回归分析方法对有关LEADDP的危险因素作了探讨。结果 本组发现LEADDP21例,占42% 。多因素分析显示对LEADDP影响最大的前二个因素依次是:空腹血糖水平升高及高密度脂蛋白-胆固醇水平降低。与糖尿病并发症的关系则显示与糖尿病足的关系最密切。结论 彩色多普勒可以对LEADDP做出早期诊断。LEADDP的发生与血糖控制不佳、血脂代谢异常相关。LEADDP在糖尿病足的发生、发展中起重要作用。Objective To investigate the diagnostic value of colour Doppler in lower extremity arterial disease in diabetic patient(LEADDP) and its risk factors.Methods The arteries of bilateral lower extremities in 50 diabetic patients and 30 normal controls were detected by colour Doppler. Multifactorial stepwise regression analysis was used to study the risk factors of LEADDP. Results 21 cases of LEADDP were found (42%). Multifactorial analysis showed the increasing FPG level was the first influential factor,and the second was the decreasing HDL C level. In the relationship between the LEADDP and other diabetic complications, diabetic foot (DF) was the most intimate.Conclusion Colour Doppler is highly sensitive, and helpful to the early diagnosis of LEADDP. The development of LEADDP is related to the control of both blood glucose level and abnormol lipid metabolism. LEADDP plays an important role in the formation and development of DF.
