摘  要:目的:分析子痫前期患者与正常妊娠晚期孕妇凝血指标及妊娠结局的差异,探讨凝血指标与子痫前期之间的关系。方法:对96例子痫前期孕妇及100例正常妊娠晚期孕妇的血常规(HB、HCT、PLT、MPV)、出凝血指标(PT、APTT、FIB)进行检测和比较,并对各组孕妇的子痫、HELLP综合征、胎盘早剥、胎儿生长受限等并发症发生率进行比较。结果:与正常孕妇相比,子痫前期孕妇的HB、PLT、PT、APTT显著降低,MPV、FIB显著增高,各种妊娠期合并症的发生率均显著增加,差异均具有统计学意义(P<0.05)。重度子痫前期较轻度子痫前期孕妇的PLT、MPV、FIB的变化更明显,子痫及HELLP综合征的发生率均显著增加,两组间差异具有统计学意义(P<0.05)。结论:子痫前期可导致多种严重妊娠结局的发生。子痫前期时血液处于高凝状态,PLT、MPV、PT、APTT、FIB等指标可用于判断患者的凝血状态及疾病的严重程度。Objective:To analyze the differences of coagulation parameters and pregnancy outcomes between cases with preeclampsia and normal women of late pregnancy,explore the relationship between coagulation parameters and preeclampsia.Methods:Blood routine and coagulation parameters of 96 cases with preeclampsia and 100 normal women of late pregnancy were tested and compared,and the incidences of eclampsia,HELLP syndrome,placental abruption,fetal growth restriction were compared between the two groups.Results:HB,PLT,PT and APTT in preeclampsia group were significantly lower than those in control group,while MPV,FIB and incidences of pregnancy complications in preeclampsia group were significantly higher than those in control group(P〈0.05).The changes of PLT,MPV and FIB in severe preeclampsia group were larger than those in mild preeclampsia group,the incidences of eclampsia and HELLP syndrome increased significantly,there was significant difference between the two groups(P〈0.05).Conclusion:Preeclampsia can lead to many serious pregnancy outcomes.Blood maintains hypercoagulable state during preeclampsia,PLT,MPV,PT,APTT and FIB can be used to assess the coagulation status and the severity of the disease.
