腹腔镜辅助阴式子宫切除与阴式子宫切除比较

摘　　要：目的：探讨腹腔镜辅助阴式子宫切除术（LAVH）与阴式子宫切除术（VH）不同的手术适应症、临床疗效及术后并发症情况。方法：对2008年1月2009年12月收治的行LAVH 100例及行VH 67例患者的临床资料进行回顾性分析,对两组患者的手术适应症、手术时间、出血量、术后肛门排气时间、下床活动时间、住院时间及术后并发症进行比较。结果：LAVH组的手术时间、术后肛门排气时间显著长于VH组,LAVH组术中出血量显著少于VH组,两组患者在下床活动时间、住院时间及术后并发症方面差异无统计学意义（P〉0.05）。结论：LAVH与LV手术效果相近,但VH的手术适应症明显的受到限制,仅适用于无粘连、子宫小的患者,LAVH扩大了VH的手术适应症,且有开腹手术的清晰视野,值得临床推广。Objective：To explore the operative indications,clinical efficacies and postoperative complications of laparoscopic assisted vaginal hysterectomy（LAVH） and vaginal hysterectomy（VH）.Methods：The clinical data of 100 cases receiving LAVH and 67 cases receiving VH from January 2006 to December 2009 were analyzed retrospectively;the operative indications,operation times,blood loss,anal exhaust times,evacuation active times,hospitalization times and postoperative complications of the two groups were compared.Results：The operation time and anal exhaust time in LAVH group were significantly longer than those in VH group,the amount of intraoperative blood loss in LAVH group was significantly less than that in VH group;there was no significant difference in evacuation active time,hospitalization time and postoperative complications between the two groups（P〉0.05）.Conclusion：The clinical efficacies of LAVH and LV are similar,but the operative indications of VH is limited,which is suitable for the cases without adhesion and with small uterus;LAVH expands the operative indications of VH,has a clear vision of abdominal surgery,which is worth popularizing.
