股骨头缺血性坏死的MRI诊断

摘　　要：目的：分析股骨头缺血性坏死的ＭＲＩ表现，评价ＭＲＩ对本病的诊断意义。方法：收集经ＭＲＩ检查诊断的１６例２６个股骨头缺血性坏死患者，常规ＳＥ序列，轴位和冠状位扫描，Ｔ１—加权和Ｔ２—加权成像，１５例经Ｘ线平片检查，３例有ＣＴ结果。结果：股骨头缺血性坏死最直观的ＭＲＩ表现是股骨头内出现异常信号，８个出现“双线征”；具有特异性；２３个股骨头塌陷，１０个髋关节狭窄，５个股骨向外上移位。结论：利用ＭＲＩ诊断股骨头缺血性坏死，能够准确地勾画出病变的范围和位置，具有高度敏感性和特异性，具有重要的临床应用价值。Objective:To evaluate the MRI imaging in the diagnosis of avascular necrosis of the femoral head (ANFH).Methods:Sixteen cases with 26 affected femoral heads with ANFH were diagnosised by MRI imaging.SE sequence on T 1 WI and T 2 WI with 2 acquisition coronal and axical imaging.15 cases were examined by X ray,and 3 cases by CT.Result:The first MRI imaging of ANFH is abnormal signal;Eight cases showed the typical ″double line sign″,Twenty three affected heads showed collapse,Ten affected hip joints showed structure and 5 femorals showed dislocation.Conclusion:MRI imaging can show the ranges and seats of the ANFH clearly.It has height sensibility and specificity,and it has important clinical value.
