耳内镜下鼓膜修补术的临床研究

摘　　要：目的:探讨耳内镜下鼓膜修补的方法、技巧和效果。方法:回顾性分析32例耳内镜下使用自体颞肌筋膜进行鼓膜修补术的临床资料。结果:患者均一次性完成手术,术后随访3～6个月,半年后复查见29例患者鼓膜穿孔完全愈合,听力提高。3例修补失败,其中2例因移植物皱缩移位有小穿孔,1例因感染,移植物脱落,治愈率为90.6%(29/32),术后半年行纯音测听检查25例语言区气骨导差缩小5～10dB,4例语言区气骨导差缩小11～15dB,无面瘫,眩晕及耳鸣等并发症发生。结论:应用耳内镜完成鼓膜修补具有操作方便、愈合率高、并发症少、节省费用等优点,值得临床广泛应用。Objective To explore the methodology,skills and curative effect of myringoplasty under otic endoscope.Methods The data of 32 cases treated by myringoplasty under otic endoscope with autologous temporal fascia were analyzed retrospectively.Results Within all the 32 cases treated after 6 months,29 tympanic perforation were cured in 32 ears（90.6%） ,and the hearing had been improved.We failed in 3 cases,among them 1 patient＇s tympanic perforation was not cured by the course of infection,and the transplantation lost.Another 2 tympanic membranes got a small perforation when the transplantation shifted.Half a year later,we found the average air-bone gap improvement for 25cases was 5～10 dB,and 11～15 dB was improved in another 4 cases.Through the procedure,no complication occurred,such as paralysis of the facial nevrve,vertigo,tinnit us,and so on.Conclusion The application of myringoplasty under otic endoscope has many advantages,we can use it conveniently and get a high cure rate with less complication,so this method deserves clinical popularization and application plus its lower expense.
