不明原因早期自然流产免疫学病因初探

摘　　要：通过观察不明原因早期自然流产患者蜕膜中淋巴细胞的种类及其表面形态学变化,分析其与该病免疫学发病因素之间的关系。以10例正常妊早期施行人工流产的健康孕妇作为对照组,采用光镜及扫描电镜综合分析的方法,对10例早期自然流产患者蜕膜组织中不同淋巴细胞的百分率及表面形态进行检测。实验组蜕膜淋巴细胞百分率为63.35±2.19％,其中大颗粒淋巴细胞及小淋巴细胞在所有蜕膜细胞中所占的百分率分别为60.05±1.92％、3.30±0.58％;而对照组脱膜淋巴细胞百分率为56.10±3.47％,其中大颗粒淋巴细胞和小淋巴细胞分别为52.35±3.42％及3.75±0.59%,实验组与对照组相比,总淋巴细胞及大颗粒淋巴细胞百分率显著上升(P<0.0005),小淋巴细胞百分率未见显著改变(P>0.05)。扫描电镜观察结果发现,不论大颗粒淋巴细胞还是小淋巴细胞,实验组均表现出细胞功能活跃的表面形态学变化,同时还可观察到,实验组呈凋亡细胞形态学变化的淋巴细胞显著增多,与对照组相比,前者为6.40±1.17％,后者为3.15±0.75%(P<0.0005)。不明原因早期自然流产患者蜕膜淋巴细胞的表面形态学发生了改变,为进一步研究该病的免疫学发病机制提供一定的实验依据。Objective:To analyse the sorts,superficial morphologic changes of decidual lymphocytes from patients with unexplained spontaneous early abortion.Methods:Ten healthy pregnancy women as the control and ten patient senrolled for the study. To observe the percentages of different lymphocytes and the surface morphology by the combined methods of lightmicroscope and scanning electron microscope. Results; The percentage of total lymphocyte in patients is higher than that of control group (63.35% + 2.19% v.s 56.10% ± 3.47%, P<0.0005),in which the proportions of largegranular lymphocytes (LGL) upregulated signififcantely (60.05% ± 1.92% v.s52.35% + 3.42%, P<0.0005),and that of small lymphocyte has no change (3.30% ± 0.58% v.s 3.75 ± 0.59% ,P>0.05).Either LGL or small lymphocyte ofpateint has surface morphological changes with active cell function, eg; various surface projections, ruffles and many microvilli with high concentration:more secretory granules and connection between cells,et al.Moreover,the percentage of apoptotic lymphocytes from patients increases greately (6.40% ± 1.17% v.s 3.15% ± 0.75%,P<0.0005). Conclusion; These data provide an experimental evidence about the decidual cellular morphology for researching immunological pathogenesis in this disease.
