摘 要:目的探索心肌组织局部和整体固有异质性变化对螺旋波稳定性的影响。方法运用计算机仿真技术模拟二维心肌组织的电生理活动,通过调整组织中主要离子电导的分布及大小达到改变组织局部和整体异质性的效果。在此基础上,观察相应螺旋波的稳定状态及衍化情况,探寻心肌组织固有异质性与螺旋波稳定性的关系。结果局部组织异质性异常:当局部异常区域位于组织中央时,螺旋波稳定性显著下降,且随着异常区域增大,螺旋波稳定性呈现进行性下降趋势;远离波头处出现局部异常组织时,螺旋波整体稳定性受影响较小。组织整体异质性异常:随着相邻异质区域尺寸的增大,螺旋波逐渐由稳定状态经游弋螺旋波、混沌游弋螺旋波直至发生螺旋波破碎。结论局部组织异质性异常:局部异常组织与螺旋波波头的位置关系对螺旋波稳定性具有重要影响。相对于远离波头的情况而言,局部异常组织靠近波头时,螺旋波的稳定性显著下降。组织整体异质性异常:组织整体异质性程度与相邻异质区域的尺寸有关,当不同区域尺寸较大时,异质性程度增高,相应螺旋波的稳定性下降。Objective To analyze the effect of local and global fixed electrophysiological heterogeneity of myocardial tissue on the stability of spiral wave.Methods The computer simulation technology was used to simulate the electrophysiological activity of myocardial tissue.The distribution and size of the conductivity of major ions were adjusted to change the local and global fixed electrophysiological heterogeneity of the tissue.On this basis,the stability of the corresponding spiral waves was observed to explore the relationship between the fixed heterogeneity and the stability of spiral wave.Results The heterogeneity of the local tissue was abnormal.When the abnormal area was located at the central part of the tissue,the stability of spiral wave significantly decreased,and the bigger the abnormal areas,the more obviously the stability of spiral wave decreased;when the abnormal part was away from the tip of the spiral wave,the global stability of spiral wave was less affected.The heterogeneity of the overall tissue changed.With the increase in the size of adjacent region,the spiral wave gradually changed from nearly stable,meandering,hypermeandering,to breakup.Conclusion The heterogeneity of the local tissue is abnormal.The positional relationship between the local abnormal part and the tip of spiral wave plays an important role in the stability of spiral wave.Compared with the result of the abnormal part away from the tip,the stability decreases more obviously with the abnormal part near the tip.The heterogeneity in the overall tissue changes.The global heterogeneity is associated with the size of the adjacent region;the bigger the size,the more the heterogeneity increases and the lower the stability of the spiral wave.
