汉化Morse跌倒评估量表在中国临床护理中应用的可行性

摘　　要：目的通过测试内部一致性、条目分析、重测信度,评价汉化Morse跌倒评估量表(CMFS)的可靠性,评价其对住院患者跌倒风险的预测能力,分析其在中国临床护理中应用的可行性。方法利用CMFS对460例住院患者进行测评,以实际发生跌倒的患者数为金标准,评价其信度、效度及预测性能;分析各条目对住院患者跌倒发生的影响。结果 CMFS简便、易测,其6个条目相互独立,界点为45分时量表的敏感度为74%、特异度为82%,对患者跌倒倾向具有较高预测能力。结论 CMFS具有较好的信度,对有跌倒风险的住院患者有一定的预测能力(量表评分≥45分的患者作为发生跌倒的重点关注人群),可以试用于中国临床护理中并进行更深入的测试。Objective To estimate the predictive power of Chinesized Morse Fall Scale（CMFS）in assessing inpatient fall risk by internal consistency,item analysis,and inter-rater and test-retest reliability,and to analyze the feasibility of CMFS in clinical nursing.Methods A total of 460 inpatients was recruited,and the fall risk was assessed by using CMFS.Data on the number of patients who fell rather than the number of falls were collected to determine the reliability,validity,and predictive power of CMFS.The effect of each item on the fall risk was analyzed.Results CMFS was a simple,convenient scale.The evaluation revealed a low Cronbach’s alpha coefficient and a low-to-moderate item-scale correlation.When the cut-off point was determined at 45,the sensitivity and specificity were 74% and 82%,respectively.CMFS had a good predictive power for falls.Conclusion CMFS has a good sensitivity and specificity,and it can be used for assessing inpatient falls（the patient should be in high risk of fall when the score is more than 45）.Further study on the applicaition of CMFS in clinical nursing is needed.
