多发性肋骨骨折的内固定治疗

摘　　要：目的对多发性肋骨骨折内固定手术治疗的手术指征、手术方法进行讨论。方法对57例多发性肋骨骨折中6例用软钢丝内固定，6例主要为单侧后外侧骨折，肋骨骨折数3～7根，其中2例胸廓塌陷、变形，但无明显胸壁软化。手术在全麻下经后外侧切口进行。结果术后胸壁稳定，胸廓塌陷畸形接近矫正．疗效满意。结论对于严重胸壁塌陷的多发性肋骨骨折应行内固定治疗，用软钢丝对部分肋骨骨折进行复位固定是简单有效的方法。Objective To value the results of internal fixation of multiple fractured ribswith soft wires and discuss the indications and the technical points of the operation. Methods Among57 cases of multiple fractured ribs 6 cases were managed by internal fixation of ribs with soft wires. Thefracture location was mainly lateral or posterolateral and the nummber of ribs broken in each pattensranged between 3 and 7, 2 cases of gross chest wall deformity but the chest wall was stable. Under general anaestheia, some of fractured ribs were fixed by soft wires through standard thoracic insicion. ResultS 3 cases flail chest wall were stabilized and 2 cases severe chest wall deformity were almost retrieved. The total fixation rate of fractured rib sites are 45%. ConClusion Severe collapsed chest wallcaused by multiple fractred ribs should be treated by internal fixation and internal fixation of some fractured ribs with soft wires is a simple and effective methods.
