摘 要:目的:研究外周血单核细胞LPS-TLR4-NF-κB通路在妊娠期糖尿病(GDM)、正常孕妇及育龄妇女外周血单核细胞的表达,探讨GDM的发病机制。方法:选取GDM孕妇、正常孕妇以及育龄妇女各20例,抽取外周静脉血5ml,分离单核细胞,用内毒素(LPS)刺激培养,RT-PCR法检测TLR4及NF-κB mRNA的表达,ELISA法检测培养液中IL-1、IL-10、TNF-α水平。结果:3组妇女TLR4、NF-κB mRNA表达的差异有统计学意义(P=0.000)。GDM组及正常孕妇组TLR4与NF-κB mRNA表达水平有良好的相关性(rGDM=0.768,r正常孕妇=0.833)。育龄妇女组TLR4与NF-κB mRNA的表达水平相关性稍差(r育龄妇女=0.719)。3组培养液中IL-1、IL-10、TNF-α水平的差异有统计学意义。结论:GDM孕妇体内TLR4、NF-κB mRNA的大量表达,提示可能LPS-TLR4-NF-κB通路介导了炎症因子释放,参与了GDM的发病。Objective:To investigate the expression of LPS-TLR4-NF-κB pathway in peripheral blood mononuclear cells of gestational diabetes,normal pregnant women and women of childbearing age and explore the pathogenesis of GDM.Methods:5ml of venous blood was obtained from 20 healthy pregnant women,20 women of childbearing age(control group) and 20 patients with gestational diabetes mellitus(GDM group).Mononuclear cells were isolated,then stimulated and cultured with LPS.TLR4 and NF-κB mRNA expression in mononuclear cells were detected by RT-PCR,and the levels of IL-1,IL-10 and TNF-α in the supernatant of medium were determined by ELISA.Results:There were statistical differences between the expression of TLR4 mRNA and NF-κB mRNA in GDM group and control group(P=0.000),and there were positive correlations between the GDM and normal pregnant women group(rGDM=0.768,rnormal pregnant women=0.833)(P0.05) or the group with women of childbearing age(r=0.719).The levels of IL-1,IL-10 and TNF-α were statistically significant in the three groups(P=0.000).Conclusion:The large number of TLR4 mRNA and NF-κB mRNA expression present immune inflammation reaction: elevated levels of the imflammatory cytokines IL-1,IL-10 and TNF-α.The inflammatory pathway of LPS-TLR4-NF-κB may be involved in the pathogenesis of GDM.
