早产及其相关危险因素的研究

摘　　要：目的：探讨早产及其相关危险因素。方法：纵向随访研究，通过面对面问卷调查，收集妊娠妇女背景资料、孕期个人行为习惯、社会环境因素、妊娠期健康状况等相关信息，随访至分娩，收集妊娠结局资料。数据采用多因素Logistic回归分析方法。结果：711例调查对象中，早产117例（16．46％），发生率较既往报道高。未发现妊娠期个人行为习惯与早产的关系，但年龄增长、工作噪音和胎膜早破与早产明显有关，OR分别为1．43（95％CI：1．05—1．94）、3．38（95％CI：1．21～9．39）、2．14（95％CI：1．19～3．83）。结论：面对早产的高发生率，不应忽略社会环境的潜在影响。对外来务工的妊娠妇女，孕期保健时应常规询问职业状况等因素，以及时发现不利影响，给予适当帮助和指导。Objective： To explore preterm delivery and relative risk factors. Methods： The information including background data of pregnant women, personnel behaviors habits during pregnancy, social - environmental factors and health status during pregnancy were col- lected by longitudinal follow - up study and face to face interviews, then the pregnant women were followed up till delivery, the data of pregnant outcomes were collected ; the data were analyzed by muhiple Logistic regression analysis. Results： Among 711 pregnant women investigated, the incidence of preterm delivery was 16.46% （117 pregnant women） , which was higher than previous reports. The relationship between personnel behaviors habits during pregnancy and preterm delivery was not found; but increasing age, occupational noise and premature rupture of membranes were related to preterm delivery closely. The OR values were 1.43 （95% CI = 1.05 - 1.94 ） , 3.38 （95% CI = 1.21 - 9. 39 ） and 2. 14 （ 95% CI = 1.19 - 3.83 ）, respectively. Conclusion ： The potential effect of society and environment should not be neglected facing the high incidence of preterm delivery, for the migrant pregnant women, occupational status should be inquired routinely during antenatal care in order to find adverse effect and provide proper help and direction in time.
