免疫球蛋白静脉注射治疗小儿重症肺炎的临床研究

摘　　要：目的观察评价静脉注射免疫球蛋白治疗小儿重症肺炎的疗效。方法将收治的110例小儿重症肺炎患者随机分为两组,治疗组和对照组各55例。对照组应用抗生素、抗病毒、吸氧、纠正心力衰竭及维持酸碱平衡等常规治疗。治疗组在对照组治疗的基础上静滴免疫球蛋白,用量0.4g·kg-1·d-1,连用3d。观察疗效、症状改善情况以及治疗前后IgG、IgA、IgM值变化。结果治疗组总有效率为94.55%,对照组的总有效率85.45%,两组间的差异有统计学意义(χ2=7.71,P<0.05)。治疗组在改善咳嗽、肺部啰音、退热、心衰等症状及缩短住院时间等方面均比对照组短,差异均有统计学意义(t分别=4.23、3.67、4.89、6.55、8.47,P均<0.05)。免疫球蛋白组在治疗后IgG与治疗前比较明显升高,差异有统计学意义(t=12.33,P<0.05),IgA、IgM治疗前后变化比较,差异均无统计学意义(t分别=0.67、0.74,P均>0.05)。两组患者均未见明显不良反应。结论免疫球蛋白静脉注射治疗小儿重症肺炎安全有效。Objective To evaluate the effect of intravenous immunoglobulin treatment in children with severe pneumonia.Methods A total of 110 pediatric cases with severe pneumonia were randomly divided into two groups：treatment group and control group（n=55）.Control group received conventional treatment such as antibiotics,anti virus,oxygen,acid-base balance and heart failure correcting.The treatment group were treated with intravenous immune globulin（0.4g·kg-1·d-1,3 days） on the basis of the control group methods.The IgG,IgA,IgM value and symptom changes were observed before and after treatment.Results The total effective rate in treatment group was 94.55%,control group was 85.45%,the difference was statistically significant（χ2=7.71,P0.05）.Cough,pulmonary rale,fever,heart failure and other symptoms and hospital stay time were obvious improved in the treatment group than that in the control group,the differences were statistically significant（t=4.23,3.67,4.89,6.55,8.47,P0.05）.IgG in the treatment group was significantly higher compared with that before treatment（t=12.33,P0.05）;the differences of IgA and IgM changes before and after treatment were not statistically significant（t=0.67,0.74,P0.05）.Patients had no obvious adverse reactions.Conclusions Intravenous immunoglobulin treatment in children with severe pneumonia is safe and effective.
