卡泊芬净与二性霉素B治疗血液病合并侵袭性真菌感染的临床研究

摘　　要：目的观察应用卡泊芬净和二性霉素B治疗血液病合并真菌感染的临床疗效及安全性。方法 37例血液病患者合并真菌感染时单用卡泊芬净13例,二性霉素B24例;24例同时应用了粒细胞集落刺激因子（G-CSF）,10例用静脉注射丙种球蛋白。以上均为单药静脉输注给药至体温正常2周或病灶消失。中位疗程30 d（7~63 d）。结果 37例中治愈率和有效率分别为51.4%和67.6%。卡泊芬净和二性霉素B组的有效率分别为69.2%和66.7%,两组间差异无统计学意义（P〉0.05）。卡泊芬净组无不良反应。二性霉素B不良反应总发生率41.7%。结论卡泊芬净和二性霉素B治疗侵袭性真菌感染的疗效较好,卡泊芬净疗效肯定且药物耐受性好。Objective To analyze the efficacy and safety of caspofungin,amphotericin B,for fungal infections in 37 patients with malignant hematologic diseases.Methods Data of 37 patients（male 21,female 16;average age 34 years）with malignant hematologic diseases in our hospital between June 2002 and December 2008 were retrospectively analyzed.Thirteen of them were given caspofungin,70 mg first day and 50 mg per day,Twenty-four of them were given amphotericin B,5~35 mg/d per day,for 7~63 days（average time 30 days）;Results The overall cure rate was 51.4%,and clinical efficacy rate was 67.6%.The clinical efficacy rate for caspofungin and amphotericin B were 69.2% and 66.7%.The incidence of adverse events was 41.7%.Conclusion Both caspofungin and amphotericin B were high-efficiency drug in treatment of systematical fungal infections.caspofungin is a relatively safe and effective drug.
