CKD并发症下血清脂联素水平的变化及临床意义

摘　　要：目的探讨慢性肾病(CKD)患者血清脂联素水平与肾功能损伤程度、CKD相关并发症之间的关系及其相应的临床价值。方法随机入选199例肾病门诊及住院的CKD患者,计算患者肌酐清除率,肾小球滤过率,并采用ELISA方法对各临床病例的血清脂联素水平进行测定。对患者的临床各相关指标进行分析,两组数据之间采用t检验、多组数据之间采用One-way ANOVA检验、多元线性回归分析。结果血清脂联素随着肾功能的降低而升高(P<0.05);伴有高血压与不伴有高血压的慢性肾病患者的脂联素水平显著不同(P<0.05),伴有糖尿病与血糖正常的慢性肾病患者相比脂联素水平无差异性(P>0.05);脂联素与慢性冠心病的24小时尿蛋白水平之间高度相关(P<0.01)。结论慢性肾病患者的血清脂联素水平随患者肾功能的丢失而显著增加;脂联素水平的变化还与CKD患者中相关并发症的发生发展存在密切的关系。Objective To investigate the change and clinical values between the serum adiponectin levels and renal injury or CKD relative complications in CKD patients. Methods To analyze some clinical indexes of 199 clinical and hospitalized CKD patients at random,calculate creatinine clearance rate and glomerular filtration rate,the serum levels of adiponectin were determined by ELISA assay. Results Serum levels of adiponectin increased with gradually renal function loss（P 0.05）;the serum adiponectin levels were significantly lower in patients with hypertension,whereas there was no difference between CKD patients with and without diabetes（P 0.05）;adiponectin was significantly correlative to 24 hour urine protein（P 0.01） . Conclusions Serum adiponectin levels increase distinguishedly with the loss of renal function in CKD patients;and the levels of adiponectin are closely related to the relative complications developing in CKD patients.
