塞莱昔布超前镇痛对跟骨内固定取出术的镇痛效果

摘　　要：目的观察塞莱昔布超前镇痛对跟骨内固定取出手术的镇痛效果和不良反应。方法 60例行跟骨内固定取出手术的患者,随机分成观察组和对照组,每组30例。观察组给药方案为"术前口服塞莱昔布胶囊400mg、术后即刻口服200mg、第1、2天每日两次、每次200mg",对照组在同样的时间给予安慰剂。服药后2小时行疼痛VAS评分,记录患者术后的睡眠满意度,塞莱昔布相关的不良反应以及盐酸哌替啶的使用量。结果术后当日、术后第1天、术后第2天的VAS值,观察组均低于对照组,差异有统计学意义。观察组的治疗有效率为93%(28/30),治疗组为47%(14/30),差异有统计学意义(P<0.05)。没有发生塞莱昔布相关的药物不良反应。结论塞莱昔布超前镇痛方案对跟骨内固定取出术的镇痛效果良好。Objective To observe the effects of preoperative celecoxib analgesia and adverse reactions on the operation of removing internal fixation of calcaneus. Methods Sixty cases of removal of internal fixation of calcaneus were randomly divided into the observation group（n=30） and the control group（n=30） . Dosage regimen for the observation group was ＂celecoxib capsules 400 mg before operation,200 mg immediately after operation,200 mg bid for the first and second days＂,while placebo were given to the control group at the same time. Pain intensity was measured by using visual-analog scale（VAS） 2 hours after drug taken and the patients＇ sleep satisfaction,the side effects of celecoxib and the added use of pethidine were recorded. Results The VAS scores of the observation group were lower than those of the control group in the day of operation,the first and the second days after operation,the difference was statistically significant（P 0.05） . The effective rate of treatment in observation group was 93%（28/30） and 47%（14/30） in treatment group,the difference was statistically significant（P 0.05） . There were no adverse reactions related to celecoxib. Conclusion Preoperative celecoxib analgesia program has good results in the operation of removing internal fixation of calcaneus.
