摘  要:目的探讨应用负压封闭引流技术治疗四肢大面积皮肤撕脱伤坏死并感染创面的临床疗效。方法对30例四肢大面积皮肤撕脱伤坏死并感染创面应用负压封闭引流治疗,术后50~60KPa中心负压吸引,连续2~3个周期治疗后对创面行中厚皮片植皮或局部皮瓣转位。结果经2~3个周期持续负压封闭引流治疗后,本组30例患者创面肉芽组织新鲜,呈粉红色颗粒状,26例患者创面直接行中厚皮片植皮,4例创面行局部皮瓣转位,创面全部愈合。结论负压封闭引流可刺激肉芽组织生长,加速创面愈合,引流充分彻底,是一种治疗四肢大面积皮肤撕脱伤坏死并感染创面的有效方法,值得临床推广。Objective To investigate the effect of application with the vacuum sealing drainage in treating necrosis and infection wound of large avulsion injury of skin.Methods Thirty patients with necrosis and infection wound of large avulsion injury of skin were treated by the vacuum sealing drainage.The negative pressure from 50 to 60 kPa was exerted to the wound.The second stage skin grafting or regional flap transposition was performed on the wound after 2 or 3 period treatments with vacuum sealing drainage.Results After 2 or 3 period treatments with vacuum sealing drainage,the granulation tissue in the surface of wound was all fresh.Twenty-six cases’ wound surface healed after second stage skin grafting with intermediate split thickness skin graft,and the rest wound healed with regional flap transposition.Conclusion The vacuum sealing drainage device can stimulate the granulation tissue growth and accelerate the wound healing.It can be considered one of the effective methods for treating necrosis and infection wound of large avulsion injury of skin and worthy to be carried out.
