摘  要:目的了解重症监护病房(ICU)鲍氏不动杆菌(ABA)的耐药变迁,为指导临床合理使用抗菌药物、控制医院感染提供依据。方法对2003年1月-2009年12月ICU分离的170株ABA,用纸片扩散法检测哌拉西林等21种抗菌药物的敏感性并对药敏试验结果进行分析。结果 7年共检出ABA 170株,对常用抗菌药物均产生了不同程度的耐药,总耐药率<50.0%的抗菌药物有头孢哌酮/舒巴坦、亚胺培南、美罗培南、米诺环素和多黏菌素E;耐药率持续<50.0%的抗菌药有头孢哌酮/舒巴坦、亚胺培南、美罗培南,对亚胺培南、美罗培南的耐药率从2003年的0、6.7%上升至2009年的75.0%、73.7%。结论 ABA的耐药率逐年上升,动态监测ICU中ABA的耐药性变迁,有助于指导临床策略性应用抗菌药物,为预防医院感染提供应对措施。OBJECTIVE To investigate the drug resistance diversity of Acinetobacter baumannii in intensive care unit in recent years,to instruct proper clinical antibacterial application and control nosocomial infection.METHODS The antimicrobial resistance of 170 strains of ABA isolates,which were collected from patients in ICU between Jan 2003 and Dec 2009 was analyzed by using Kirby-Bauer method to test the antimicrobial susceptibility to 21 kinds of antibacterial and the testing results were analyzed.RESULTS A total of 170 strains of ABA isolates were collected in 7 years.ABA were resistant to commonly used antibiotics in different degrees.The antibacterials with general drug resistance rate below 50.0% were cefoperazone/sulbactam,imipenem,meropenem,minocyline and colistin.The antibacterial with drug resistance rate 50% for successively 7 years were: CSL,IPN and MEM.The resistance rates of IPM and MEM had been increased from 0 and 6.7% in 2003 to 75.0% and 73.7% in 2009.CONCLUSION The resistance rate of ABA is rising year by year.Dynamic monitoring of the change of the drug resistance of ABA in ICU can help to guide the strategic selection of antibiotics and to take effective measures to prevent the hospital infection.
