脑膜脓毒金黄杆菌耐药性分析及金属β-内酰胺酶基因型研究

摘　　要：目的了解临床分离脑膜脓毒金黄杆菌的耐药性,并对其产金属β-内酰胺酶的产酶率及基因型进行分析。方法运用K-B法对49株脑膜脓毒金黄杆菌进行药敏试验检测,并用三维试验、改良三维试验和2-巯基丙酸抑制试验检测金属β-内酰胺酶,采用聚合酶链反应(PCR)及测序分析,确定金属β-内酰胺酶的基因型。结果 49株脑膜脓毒金黄杆菌对米诺环素的耐药率最低为6.1%,对哌拉西林/他唑巴坦的耐药率为30.6%,对头孢哌酮/舒巴坦的耐药率为34.7%,其余抗菌药物的耐药率均>50.0%;用三维试验、改良三维试验和2-巯基丙酸抑制试验检测到33株菌株产金属β-内酰胺酶,产酶率为67.3%,33株细菌均扩增到金属β-内酰胺酶基因,其中29株blaB基因,21株为blaGOB基因。结论脑膜脓毒金黄杆菌的高耐药率与高产酶率有关,主要基因型为blaB及blaGOB,发生该菌感染时,临床医师应根据病情,参照实验室药敏结果选用合适的抗菌药物。OBJECTIVE To understand the producting enzyme rate and the antibiotical resistance of the clinical isolated Chryseobacterium meningosepticum,and to analyze the genotype of metal-β-lactamase（MBL）.METHODS The susceptibility test of 49 C.meningosepticum strains were detected by K-B method,and phenotype of MBL was detected by three dimension test,modified three dimension test and 2-mercaptopropionic acid inhibitory test.Polymerase chain reaction（PCR） amplification with some prime sets for MBL gene was conducted for all isolates.RESULTS The drug resistance rate of 49 C.meningosepticum strains to minocycline was 6.1%,to piperacillin/tazobactam and cefoperazone/sulbactam was 30.6%,and 34.7%,respectively,and to other antimicrobials was more than 50.0%.Thirty-three strains were identified and confirmed to produce MBL using three dimension test,modified three dimension test and 2-mercaptopropionic acid inhibitory test.The MBL producing rate of all isolates was 67.3%,of which 33 isolates were detected to have MBL by PCR amplification.Among them,29 strains were detected to have blaB type and 21 strains to have blaGOB type.CONCLUSIONS The high resistance rate of C.meningosepticum may be due to the high producing rate of β-lactamases.It is important for selecting the antimicrobial drugs rationally according to illness state and susceptibility test results to treat C.meningosepticum infection.
