不同给药时间对非杓型高血压病患者血压影响的探讨

摘　　要：目的通过临床观察和指导用药获得合理有效的给药时间,从而有效控制非杓型高血压患者血压水平。方法对30例非杓型高血压患者采用自身对照法,分别采用不同时间给予Ca2+通道阻滞剂和ACEI类药物进行治疗。先清晨服用Ca2+通道阻滞剂和ACEI类长效制剂,随后改为清晨(6:00～7:00)服用Ca2+通道阻滞剂,晚上(20:00～21:00)服用ACEI类长效制剂。两种给药方式均在实施4周后进行24h动态血压监测。结果分开给药后患者血压均得到了有效控制,分时段给药后24h平均压,nDBP,夜间平均压,dDBP,白昼平均压都显著降低,自身血压比较差异有显著意义(P<0.05)。结论采用分时段给药可以达到有效平稳降压,从而有助于保护靶器官的结构和功能,最大程度地减少高血压相关并发症的发生。Objective To obtain the most rational and effective administration time through clinical observation and instructive administration of drug,and to control the level of blood pressure for patients with non-dipper hypertension correspondingly.Method 30patients with non-dipper hypertension was selected by the non-random self control method and treated by Ca2＋channel inhibitor and ACEI medicines at different time.In one method,both Ca2＋ channel inhibitor and ACEI long acting pharmaceuticals were taken by the patients in the morning;in the other method,Ca2＋channel inhibitor was taken in the morning（6：00-7：00am）and ACEI medicines at night（8：009：00pm）.Both methods for administration of drug were monitored by the patients＇24-hour dynamic blood pressure after 4weeks.Result All patients＇blood pressure were efficiently controlled by separate administration of drug,the 24-hour average pressure,nDBP,the night average pressure,dDBP,and the day average pressure of patients reduced remarkably.The comparison of self blood pressure was significant different between group（P0.05）.Conclusion The blood pressure was reduced efficiently and stably by separate administration of drug in different time slots.Consequently,the structure and function of target organs were protected and the occurrence of complications of high blood pressure was reduced to a great extent.
