99mTc-MIBI SPECT心肌灌注断层显像在心脏X综合征诊断中的价值

摘　　要：目的探讨甲氧基异丁基异晴(99mTc-MIBI)SPECT心肌灌注断层显像(MPT)在心脏X综合征诊断中的临床价值。方法应用99mTc-MIBI运动与静息心肌灌注断层显像侧壁/室间隔计数比值(L/S)和运动L/S与静息L/S之再比值的方法,对30例冠状动脉造影正常、运动试验阴性的患者(对照组)与27例心脏X综合征患者(心脏X综合征组)及26例有心绞痛且冠状动脉造影阳性患者(冠心病组)的心肌显像表现进行比较。结果①心脏X综合征组、冠心病组MPT显像异常的阳性率(69.2%、92.3%)明显高于对照组(10.0%)(P<0.05);冠心病组MPT显像异常的阳性率明显高于心脏X综合征组(P<0.05)。②心脏X综合征组、冠心病组MPT显像异常的阳性患者运动L/S与静息L/S之再比值明显低于对照组(P<0.05);冠心病组运动L/S与静息L/S之再比值明显低于心脏X综合征组(P<0.05)。结论 SPECT显像的异常现象(心肌缺血及"反向再分布")及运动L/S与静息L/S之再比值减低对于心脏X综合征的诊断具有一定的临床意义。Objective To evaluate the clinical role of 99mTc-MIBI myocardial perfusion tomography（MPT） in the diagnosis of cardiac syndrome X.Methods A total of 83 patients were divided into three groups：normal coronary angiography group（n=30,with positive exercise test）,cardiac syndrome X group（n=27）,and angina pectoris group（n=26）,with positive coronary angiography）.The ratio of septal to lateral wall（L/S） at stress and rest and the ratio of stress L/S to rest L/S were compared among the three groups.Results The rate of abnormal MPT in cardiac syndrome X group and angina pectoris group（69.2% and 92.3%,respectively） was higher than that in normal coronary angiography group（10.0%）（P〈0.05）,and there was a significant difference between cardiac syndrome X group and angina pectoris group.The ratio of stress L/S to rest L/S in patients with abnormal MPT in cardiac syndrome X group and angina pectoris group was significantly lower than that in normal coronary angiography group（P〈0.05）,and there was a obvious decrease in angina pectoris group compared with cardiac syndrome X group.Conclusion The abnormal 99mTc-MIBI SPECT myocardial perfusion imaging and the decrease in ratio of stress L/S to rest L/S have clinical significance in diagnosis of cardiac syndrome X.
