摘 要:目的研究慢性阻塞性肺疾病(COPD)患者改良BODE评分与传统BODE评分的相关性。方法对2006年9月至2007年3月广州医学院第一附属医院和广东省人民医院呼吸门诊就诊的42例中、重度COPD缓解期患者进行常规肺功能、6min步行距离、MRC呼吸困难评分、体质量指数测量和症状限制递增功率心肺运动试验。改良BODE评分以最大摄氧量(VO2max)或最大摄氧量占预计值百分比(V02maxpred)代替传统BODE评分中6min步行距离。结果6min步行距离与V02max呈中度负相关(r=-0.502,P〈0.01),与V02maxpred呈低度负相关(r=-0.375,P〈O.05);传统BODE评分分别与W02max和V02maxpred呈中度负相关(r=-0.498,r=-0.556,P〈0.01);改良BODE评分1[(4.75±2.03)分]和改良BODE评分2[(4.12±2.11)分]分别与传统BODE评分[(3.67±2.01)分]呈高度正相关(r=0.945,r=0.943,P〈O.01)。结论改良BODE评分与传统BODE评分呈高度正相关,且传统BODE评分简便易行,不需要昂贵检查设备,适合临床推广应用。Objective To evaluate the correlation between the values of the modified BODE index and conventional BODE index in COPD patients. Methods Forty- two patients with moderate to severe COPD registered to Respiratory Medicine Clinics of The First Affiliated Hospital of Guangzhou Medical College and Guangdong Provincial People' s Hospital between September 2006 and March 2007, underwent routine pulmonary function test, 6- minute walk test (6MWT) , MRC dyspnoea score, body mass index (BMI) and symptom- limited incremental cardiopulmonary exercise testing. Maximal oxygen uptake (VO2max) or percentage predicted value of VO2max (VO2max%pred) was used in modified BODE index as a substitute for 6MWT in conventional BODE index. Results There was a moderately negative correlation between 6MWT and VO2max (r =-0.502, P〈O.01 ) , and a mildly negative correlation between 6MWT and VO2max%pred (r=-0.375, P〈0.05). The conventional BODE index had a moderately negative correlation with VO2max (r= -0.498, P〈0.01) and VO2max%pred (r =-0.556, P〈0.01). The modified BODE index value 1 (4.75±2.03, r = 0.945, P〈0.01 ) and value 2 (4.12±2.11, r = 0.943, P〈0.01 ) showed highly positive correlations with conventional BODE index. Conclusion The excellent positive correlation between modified BODE index and the conventional BODE index as well as the easy-to-use nature of conventional BODE index warrants its widespread use in clinical practices.
