摘  要:目的 探讨CT灌注成像观察恶性骨肌肿瘤微血管不均衡分布模式的应用价值.方法 对20例病人的20例恶性骨肌肿瘤行CT灌注检查, 比较肿瘤外周区与中央区的时间-密度曲线(TDC)和灌注参数.术后病理标本常规组织切片及SP免疫组织化学检查,单克隆抗CD34抗体标记血管,用Weidner技术计算肿瘤微血管密度(MVD).比较肿瘤外周区与中央区的MVD,分析灌注参数与肿瘤MVD的相关性.结果 20例恶性骨肌肿瘤均表现为高灌注,BF和BV图像上均可清楚显示肿瘤;肿瘤外周区与中央区的最大强化速率(MER)和MVD存在显著差异(P〈0.05),灌注参数与MVD呈正线性相关.结论 CT灌注成像可以无创地反映恶性骨肌肿瘤具有丰富的周边血管.Objective To investigate the value of the CT perfusion imaging in assessing the pattern of microvessel asymmetry distribution of malignant musculoskeletal tumors. Methods Twenty patients with malignant musculoskeletal tumors were examined with CT perfusion imaging. The difference of the time--density curve{TDC)and the CT perfusion parameters between the rim and the center of malignant musculoskeletal tumors were compared. The specimens of malignant musculoskeletal tumors were examined by histology and SP immunohistochemistry staining. The anti--CD34 monoctonal antibodies were used to mark the mierovessels. The microvessel density {MVD} of the tumors were calculated using Weidner technique, The differences of MVD between the rim and the center of malignant musculoskeletal tumors were analysed. The correlations between the CT perfusion parameters and MVD of the tumors were analysed. Results Twenty malignant musculoskeletal tumors demonstrated high perfusion with well-defined the contour on the BF and BV mapping images. The maximal enhancement rate (MERI and the MVD of the rim and the center of tumors was differed significantly{ P^0.05 ). There was positive linear correlation between the tumor's MVD and the CT perfusion parameterst. Conclusion CT perfusion imaging can reflect the rich rim microvessel of malignant musculoskeletal tumors safely.
