摘 要:目的调查我院围手术期预防性使用抗生素情况,制定管理策略,指导临床合理用药。方法选取我院手术患者658例,对围手术期预防性使用抗生素情况进行回顾性分析并进行合理性评价。结果我院围手术期预防性抗菌药物使用率为92.4%,其中Ⅰ类切口72.9%;Ⅱ、Ⅲ类切口均为100%。术前或术中使用抗生素所占的比例偏低,Ⅰ类切口抗生素术前使用率为20.5%,Ⅱ类切口31.6%,Ⅲ类切口39.5%;术后抗生素使用率在各类切口所占的比例均较高,且用药时间过长。结论我院围手术期预防性使用抗生素存在用药指征过宽、用药时机不当、选择抗生素起点过高、病原学检测率低等不足,必须加强管理,合理使用抗生素。Objective To investigate the antibiotics using during perioperative period in our hospital and to formulate management strat- egy to guide reasonable drug using in clinics. Methods The antibiotics using of 658 patients with operation during perioperative period were investigated and evaluated rationally. Results The using rate of antibiotics during perioperative period in our hospital was 92.4%, including 72.9% for I class incision, 100% for Ⅱ or Ⅲ class incision. The using rate of antibiotics before or during operation was lower, including 20.5% for I class incision, 31.6% and 39.5% for Ⅱ and Ⅲ class incision respectively. However, the using rate of antibiotics after operation was higher and the duration was longer. Lots of irrationalities in antibiotics using during perioperative period in our hospital were found. Conclusion There were some deficiencies in antibiotics using during perioperative period in our hospital including wide medication indication, non-appropriate medication opportunity, higher starting point of selecting antibiotics and low detection rate of etiology. It' s necessary to strengthen the management and to use antibiotics rationally.
