新发青中年高血压病人动态血压水平及昼夜节律  

Ambulatory Blood Pressure Levels and Blood Pressure Circadian Rhythm in Newly-onset Young and Middle-aged Patients with Essential Hypertension

摘　　要：目的探讨新发青年和中年高血压病人的动态血压水平及昼夜节律。方法对120例新发的青年和中年高血压病人行动态血压监测。120例高血压病人按年龄分为青年组(56例)和中年组(64例),观察各组病人动态血压水平及血压形态。结果新发青中年高血压病人中,血压水平属2级者占多数,需要联合用药;青年高血压病人舒张压水平高于中年病人(P<0.05),青年高血压病人心率较中年病人快(P<0.05);中年高血压病人脉压及脉压指数大于青年病人(P<0.05),且非杓型血压形态更多见。结论对于新发的青中年高血压病人,除了诊室血压测量和家庭自测血压,24h动态血压监测可以全面了解病人血压水平及血压形态,为指导临床选择降压药物品种及服药时间提供依据。Objective To investigate the ambulatory blood pressure levels and blood pressure circadian rhythm in newly-onset young and middle-aged patients（pts） with essential hypertension.Methods One hundred twenty newly-onset young and middle-aged essential hypertensive pts were categorized into two groups by age：Young essential hypertensive group （n=56） and middle-aged essential hypertensive group（n=64）.Ambulatory blood pressure was measured to find out the level and circadian rhythm for every patient.Results The blood pressure levels in most newly-onset young and middle-aged hypertensive pts was at grade 2.Young essential hypertensive pts had higher diastolic blood pressure and quicker heart rate than that in middle-aged hypertensive pts（P0.05）.Middle-aged hypertensive pts had higher blood pressure pulse and blood pressure pulse index than that in young essential hypertensive pts（P0.05）.Non-dipper blood pressure was seen in the majority in the middle-aged hypertensive pts.Conclusion In newly-onset young and middle-aged hypertensive pts,24-hour blood pressure monitoring could help to master the level of blood pressure and circadian rhythm,which would contribute to the choice of the anti-hypertensive drugs and time.
