创伤后分离体验与心理健康水平相关分析

摘　　要：目的探讨经历创伤事件后个体心理健康状况并初步了解创伤后个体分离体验与其心理健康水平之间的关系。方法对2009年9月～2010年6月间在上海市仁济医院急诊神经外科收治的60例交通事故、生产事故伤员或经历暴力事件者,分别用PDEQ、SCL-90量表对其经历创伤事件后的分离体验和总的心理健康水平进行初步评估,并对二者进行相关分析。结果经历创伤事件者PDEQ均分为0.79±1.02,且23.33%个体表现为中重度分离体验;SCL-90均分为1.56±0.53,且敌对、焦虑、躯体化、人际关系、恐怖因子均分与全国常模相比有显著差异。PDEQ得分与SCL-90中除敌对因子外的所有因子均显著相关,且与强迫因子相关性最强(r=0.446,P<0.001)。结论经历创伤事件后出现分离体验和心理症状是一种常见现象,且分离体验与个体创伤后的心理健康水平密切相关。因此,我们有必要对经历创伤事件者的分离体验予以关注,为开展有效的心理干预,减轻心理症状提供帮助。Objective To investigate the state of psychological health of subjects who have experienced the traumatic event and to explore the relationship between peritraumatic dissociative experiences and peritraumatic psychological healthy status. Methods The peritraumatic dissociative experiences questionnaire（PDEQ）and the symptom checklist-90（SCL-90） were used to assess the peritraumatic dissociative experiences and psychological wellness among 60 people who experienced traffic accidents or industrial accident or violence. Results The median of PDEQ score was 0.79±1.02, and 23.33% of the subjects showed moderate-severe peritraumatic dissociative experiences. The median of SCL-90 score was 1.56±0.53. There were significant differences in factor score of hostility, anxiety, somatization, interpersonal relationship, phobia between subjects and the national norms. There was significant correlation between the score of PDEQ and all factors of SCL- 90 but hostility, especially the obsessive factor（r=0. 446,P〈0. 001）. Conclusion It is common that some people may suffer dissociative experiences and psychological disorders after traumatic events. The dissociative experiences correlate with the peritraumatic healthy status. It is neccessary to focus on peritraumatic dissociative experiences to enable us to take effective psychological interventions and relieve psychological symptoms.
