摘 要:目的 根据羊水中板层小体计数了解胎儿肺成熟度,以判定是否终止妊娠,减少新生儿呼吸窘迫综合征的发生.方法 选择在陕西省宝鸡市妇幼保健院产科住院不同孕周的孕妇97例,经腹抽取羊水,应用KX-21N血细胞计数仪行板层小体计数,确定胎肺成熟度.结果 随着孕周的增加,板层小体的数目也有所增加.当羊水板层小体计数〈36×10^9/L时,发生新生儿呼吸窘迫综合征为76.7%(23/30),当羊水板层小体计数≥36×10^9/L时,无新生儿呼吸窘迫综合征发生(χ^2=67.33,P〈0.01).羊水板层小体计数与羊水泡沫试验经统计学分析,两者结果无显著性差异(χ^2=0.03,P〉0.05).结论 通过羊水板层小体计数预测胎肺成熟度和新生儿呼吸窘迫综合征可作为胎肺成熟度的筛查试验.Objective To count lamellar body (LB) in amniotic fluid so as to determine whether the pregnancy should be terminated and to reduce incidence of neonatal respiratory distress syndrome ( NRDS ). Methods Amniotic fluid of 97 pregnant women in different gestational ages admitted to Department of Obstetrics of Baoji Municipal Maternal and Child Health Hospital, Shaanxi Province was transabdominally extracted. The blood cell counts instrument KX-21N was used to count LB in amniotic fluid to determine fetal lung maturity. Results With increase in gestational age, count of LB in amniotic fluid was increased. When the amniotic fluid LB count was 〈36×10^9/L, the incidence of NRDS was 76.7% (23/30), when the amniotic fluid LB count was ≥36×10^9/L, no NRDS occurred (χ^2 = 67.33, P 〈 0.01 ). In predicting incidence of NRDS and fetal lung maturity, there was no significant difference between the two methods- LB count in amniotic fluid and amniotic fluid foam test (χ^2 = 0.03, P 〉 0.05 ). Conclusion Amniotic fluid LB count can predict fetal lung maturity and incidence of NRDS. So, it can be used as a screening method of fetal lung maturity.
