成年人胰岛素抵抗与多种代谢异常关系

摘　　要：目的分析吉林省延边地区成人胰岛素抵抗与多种代谢异常的相关性。方法以延边地区常住成年居民1 235人作为研究对象,进行体格检查及生化指标检测;用放射免疫法检测,空腹胰岛素,采用稳态模式评估法(HO-MA)估测人体胰岛素抵抗(IR)。结果延边地区成人的胰岛素抵抗检出率为25.0%;高血压、血脂异常、高血糖、整体型肥胖及腹型肥胖患者所占百分比随着IR水平增高而上升(P<0.0 1),且其在IR者中的比例分别为49.5%,32.4%,30.4%,68.0%及82.5%;1种代谢异常者的1 8.8%伴发胰岛素抵抗,2种代谢异常者的32.7%伴胰岛素抵抗,3种代谢异常者的42.6%伴胰岛素抵抗,4种代谢异常者的68.2%伴胰岛素抵抗;本地区IR与MS、高血糖、高血压、血脂异常、整体型肥胖及腹型肥胖均有密切的关系。结论胰岛素抵抗者伴发常见代谢异常病的现状普遍存在。不同代谢异常及其不同组合成分伴发胰岛素抵抗的频率有差异;IR与MS、高血糖、高血压、血脂异常、整体型肥胖及腹型肥胖均有密切的关系。Objective To analyze the relationship between adult insulin resistance（IR） and multiple meatbolic disorders among adults in Yanbian area.Methods A total of 1 235 adults were selected,and the anthropometric and biochemical indexes were measured.The fasting insulin（FIns） was detected with radioimmunoassay,and human IR was estimated with homeostasis model assessment（HOMA） method.Results The proportion of IR was 25.0%.The prevalence of hypertension,dyslipidemia,hyperglycemia,overall obesity,and abdominal obesity increased along with the increase of IR level（P0.01） and the ratios of the metabolic disorders in the subjects with IR was 49.5%,32.4%,30.4%,68.0%,and 82.5%,respectively.Among the adults with one kind of abnormal metabolism,18.8% were IR and the rate of IR was 32.7% among the adults with two kinds of abnormal metabolism,42.6% among those with three,and 68.2% with four kinds of abnormal metabolism,respectively.IR was closely related with hyperglycemia,hypertension,dyslipidemia,overall obesity,and abdominal obesity in the adults.Conclusion The insulin resistace with common dysbolism is ubiquity,and the ratio of different metabolic disorders and its different compositions with IR are different.IR is closely related with hyperglycemia,hypertension,dyslipidemia,overall boesity,and abdominal boesity.
