三种鼻饲方式对ICU危重症患者营养状况影响的临床研究

摘　　要：目的:探讨适合ICU危重症患者的最佳鼻饲方式。方法:将180例ICU危重症患者随机分到每日鼻饲时间持续24h泵入组、持续16h泵入组和间断24h泵入组,每组60例,持续24h泵入组和持续16h泵入组用输液泵持续泵注营养液,间断24h泵入组用输液泵分8个时间单元泵入,每个时间单元为3h,即持续泵入2h,间隔1h。结果:①三种鼻饲方式维持ICU患者营养状况的作用基本相同,但三组营养指标均有降低,提示要及早给予肠内营养。间断24h泵入法在维持血红蛋白浓度方面比持续24h泵入法差。②三种鼻饲方式均能很好地维护ICU患者的免疫力。③持续16h泵入法能保证ICU患者的充足睡眠。结论:持续16h泵入法更适合ICU患者。Objective： To explore the optimal nasal feeding pattern for ICU patients.Methods： A total of 180 ICU patients were randomly assigned into three groups,each group has 60 cases.The patients who were in the continuous 24-hour infusion group and the continuous 16-hour infusion group received continuous nasal feeding nutrient solution through infusion pump for 24 hours or 16 hours.Patients in the intermittent 24-hour infusion group received nasal feeding through infusion pump for 2 hours,then 1 hour break for one cycle,totally 8 cycles a day.Results： ① three kinds of nasal feeding patterns played the same role in maintenance of patients＇ nutritional status,but the nutritional indicators of all three groups decreased.It was suggested that enteral nutrition should begin as soon as possible in ICU patients.The intermittent 24-hour pump method is worse than the continuous 24-hour pump method in the maintenance of hemoglobin level.② All three kinds of nasal feeding patterns could maintain ICU patients＇ immunity status.③ It ensured ICU patient＇s sleep quality by the continuous 16-hour pump method.Conclusion： The continuous 16-hour infusion might be the best nasal feeding pattern for ICU patient.
