摘 要:目的:探讨引起腹泻的病原微生物种类和构成特点及其流行情况。方法:采集腹泻患者粪便,进行致泻性大肠埃希菌、沙门菌、志贺菌、空肠弯曲菌和弧菌培养鉴定和血清定型检测;对其中水样便且病原菌检测阴性的标本进行轮状病毒、诺沃克病毒、肠道腺病毒和星状病毒荧光PCR检测。结果:在2 627份粪便标本中,共检出肠道病原菌257株,其中致泻性大肠埃希菌142株(55.3%),副溶血弧菌97株(37.7%),沙门菌17株(6.6%),志贺菌1株(0.4%);未检出霍乱弧菌和空场弯曲菌;5份标本同时检出2种病原菌。在668份病原菌检测阴性的水样便标本中,有274份检出肠道病毒,其中诺沃克病毒180份(65.7%),轮状病毒77份(28.1%),星状病毒15份(5.5%)和肠道腺病毒2份(0.7%);4份同时检出轮状病毒和诺沃克病毒。结论:腹泻病原微生物类型复杂多样,以致泻性大肠埃希菌和诺沃克病毒感染居多,加强腹泻病原微生物的常规检验对腹泻病的病因诊断和治疗有重要临床价值。Objective To discuss germ's types,features and epidemics of diarrhea disease.Methods Various pathogenic bacteria including lapactic Escherichia coli,Salmonella,Shigella,campylobacter jejuni and Vibrio cholera,were isolated and identified from diarrhea stool samples through enrich culture,separate culture,biochemistry,serology,etc.The pathogenic virus such as rotavirus,Norwalk virus,enteral adenovirus and astovirus were tested with fluorescence PCR.Results In 2 627 samples,257 pathogenic bacteria were detected,in which there were 142(55.3%)strains of lapactic Escherichia coli,97(37.7%) strains of Vibrio parahemolyticus,17(6.7%) strains of Salmonella,1 strain of Shigella,and 0 strain of Campylobacter jejuni and Vibrio cholera.Five samples were detected two disease germs simultaneously.In 668 diarrhea stool samples without disease germs,274 samples were detected disease viruse,in which there were 77 samples of rotavirus,180 samples of Norwalk virus,2 samples of adenovirus,and 17 samples of astrovirus.Four samples were detected rotavirus and Norwalk virus simultaneously.Conclusion The pathogenic microorganism types of diarrhea are various and complex.The detectable rates of lapactic Escherichia coli and Norwalk virus are very high.It plays an important role to detect diarrhea pathogenic microorganism in the diagnosis and therapy.
