平衡针灸对脑卒中偏瘫患者步行能力的影响

摘　　要：目的：探讨平衡针灸对恢复期脑卒中患者步行能力的影响.方法：将具有一定步行能力的本院康复科神经内科80例脑卒中患者,随机分为治疗组和对照组,各40例.对照组患者采用常规康复训练方法,治疗组在此基础上,增加平衡针灸治疗.治疗前和治疗3周后分别评定患者的下肢运动功能、平衡功能、步行能力,以及日常生活活动（ADL）能力.结果：两组患者的下肢运动功能、平衡功能、步行能力和ADL能力治疗后均有改善（P〈0.05）,治疗组患者改良巴氏指数（MBI）为 79.41±5.85,Fugl-Meyer运动功能评定（FMA）为29.12±3.12;Berg平衡量表（BBS）为40.42±4.87,患者10米步行所需时间为（41.89±7.17）s,患者6 min内步行的距离为（41.08±2.96）米;②Holden步行能力评定治患者的Holden步行能力评定达3~5分者共32例（80.0%）,治疗组患者的效果优于对照组（P〈0.05）.结论：平衡针灸疗法可改善恢复期脑卒中偏瘫患者的运动功能、平衡功能和ADL能力,提高其步行能力.Objective：To investigate the effects of balance acupuncture on walking ability of stroke hemiplegic patients. Method： 80 stroke patients with a certain walking ability were randomly divided into treatment group and control group, with 40 cases in each group. Control group patients underwent conventional rehabilitation, and patients in treatment group were treated with balance acupuncture besides traditional rehabilitation. Lower extremity motor function,equilibrium function, ambulation capacity and the ability of daily living （ADL） in all patients were assessed before operation and 3 weeks later after operation. Result： After treatment, motor function of lower extremity, equilibrium function, ambulation capability and ADL of the patients in both groups were improved （ P 〈 0. 05 ）. In treatment group, MBI was 79.41 ± 5.85, FMA was 29.12± 3.12, BBS was 40.42 ± 4.87, the time for a lOm walk was （41.89 ± 7.17 ）s, walking distance within 6min was （41.08 ± 2.96）m. According to Holden ambulation capability evaluation, there were 32 cases （80.0%） got mark from 3 to 5. Therapeutic effect of the treatment group was better than the control group （ P 〈 0.05 ）. Conclusion： Balance acupuncture may improve motor function, balance function, ADL capacity and ambulation capability in stroke hemiplegic patients.
