实时三维超声心动图评价房、室间隔缺损患者左室心肌质量的研究

摘　　要：目的探讨实时三维超声心动图(RT-3DE)评价房间隔缺损(ASD)和室间隔缺损(VSD)患者左室心肌质量(LVM)的准确性、优越性及临床应用价值。方法对31例ASD患者、33例VSD患者及31例健康志愿者分别用M型超声(ME)、二维超声(2DE)及RT 3DE测量左室心肌质量指数(LVMI),同时用RT 3DE测量所有受试者的左室舒张末容积指数(LVEDVI)、左室每搏指数(LVSVI)及左室射血分数(LVEF)。结果 (1)RT 3DE所测LVMI小于ME测值而大于2DE测值(均P<0.05)。(2)ASD患者组LVMI、LVEDVI、LVSVI及LVEF较正常对照组偏小(均P<0.05);LVMI与LVEDVI、LVSVI、LVEF均成正相关(r=0.502～0.689,均P<0.05)。(3)VSD患者组LVEDVI、LVSVI较正常对照组偏大(均P<0.05);LVMI与LVEDVI、LVSVI成正相关(r=0.451～0.463,均P<0.05)。结论 RT-3DE能准确评估ASD、VSD患者的左室心肌质量,对研究先天性心脏病患者的心肌改变有重要指导意义。Objective To evaluate the accuracy and superiority of real-time three-dimensional echocardiography （RT-3DE）in the assessment of left ventricular mass （LVM）in patients with atrial septal defect（ASD）and ventricular septal defect （VSD）, and to explore the clinical value. Methods M mode echocardiography （ME）, two dimensional echocardiography（2DE）simpson method and RT-3DE examination were performed on 31 ASD patients, 33 VSD patients and 31 healthy volunteers respectively to determine LVMI. Meanwhile,RT-3DE was used to survey left ventricu- lar end-diastolic volume index（LVEDVI）, left ventricular stroke volume index（LVSVI） and left ventrieular ejection fraction（LVEF）. Results （1）LVMI measured by RT-3DE was less than that mea-sured by ME but larger than that measured by 2DE（all P〈0.05）. （2）ASD case： LVMI, LVEDVI, LVSVI and LVEF measured by RT-3DE were less than those in healthy volunteers（all P〈0.05）. LVMI was positively related to LVEDVI, LVSVI and LVEF（r= 0. 602-0. 789,all P〈0.05）. （3）VSD cases：LVEDVI and LVSVI measured by RT-3DE were larger than in healthy volunteers （ both P〈0.05）. LVMI was positively related to LVEDVI and LVSVI （r = 0.451 - 0. 463, both P〈 0.05 ）. Conclusions RT-3DE is a new,rapid and reliable means to assess LVM,and is of great significance to study cardiac muscle change in congenital heart disease patients.
