摘 要:目的评估肾小球滤过率(GFR)估算公式在中国糖尿病人群中适用性。方法以99mTc-DTPA同位素测定肾小球滤过率为参考标准,分别用CG公式、简化的MDRD公式估算GFR(eGFR),与同位素测定GFR结果进行比较。结果 217例糖尿病患者,年龄(56.73±13.67)岁,体重(68.13±10.88)kg,体重指数(24.69±3.49)kg/m2,血肌酐(85.64±66.33)μmol/L,尿白蛋白与肌酐比值(ACR)20.28(7.49,142.31)mg/g。同位素测定GFR(83.44±26.52)ml/(min·1.73m2),CG公式估算GFR(97.41±42.06)ml/(min·1.73m2),简化MDRD公式估算GFR(101.64±39.43)ml/(min·1.73m2)。CG估算值与GFR的偏差中位数-6.30(-31.66~5.77)ml/(min·1.73m2),MDRD估算值与GFR的偏差中位数-15.22(-34.91~0.36)ml/(min·1.73m2)。相关分析表明同位素测定的GFR与CG-eGFR和MDRD-eG-FR具有较好的相关性(r=0.65,r=0.73,P<0.01),与年龄、体重指数、血肌酐Cr、ACR有相关性(P<0.01)。一致性检验分析表明,CG-eGFR公式Kappa指数为0.515,总体符合率84.72%;简化的MDRD-eGFR公式Kappa指数为0.613,总体符合率88.42%。结论我国糖尿病人群CG和MDRD公式估算GFR与实际GFR相比具有较好的适用性。Objective To evaluate modification of the Cockcroft-Gault(C-G) formula and modification of diet in renal disease(MDRD) of glomerular filtration rate(GFR) in diabetic patients.Methods In 217 diabetic patients,we compared the C-G formula and MDRD equation estimations to GFRs measured by a radiolabeled diethlenetriaminepentaacetic acid(99mTc-DTPA) by pair t statistics,correlation studies and Kappa statistics.Results The measured GFR was(83.44 ± 26.52) ml/(min·1.73 m2) .The eGFR from C-G formula was(97.41 ± 42.06) ml/(min· 1.73 m2) .The eGFR from MDRD equation was(101.64 ± 39.43) ml/(min·1.73 m2) .Both the C-G formula(r = 0.65,P 0.01) and MDRD equation(r = 0.83,P 0.01) were well correlated with isotopic GFR.The median biaswas-6 .3 0 (3 1 .6 6 ~ 5 .7 7 ) ml / (min ·1 .7 3 m2 ) between C-G formula and measured GFR ,and was-15.22(34.91 ~ 0.36) ml/(min·1.73 m2) between MDRD equation and measured GFR.For C-G formula,the Kappa index was 0.515;For MDRD equation the Kappa index was 0.613.Conclusion Although both equations have imperfections,the MDRD equation and C-G formula have more applicability than measured GFR in diabetic patients GFR in China.
