Investigation of allergic substances in 44 facial dermatitis patients with unconfirmed reason-analysis the results of patch tests on European standard combined and cosmetic related allergens

摘　　要：目的:对面部不明原因皮炎患者的过敏物质进行分析。方法:对44例面部不明原因皮炎患者进行欧洲标准变应原联合化妆品相关变应原斑贴试验,按照国际接触性皮炎研究组推荐的标准为依据。结果:阳性率较高的欧洲标准变应原前3位均是金属类,即氯化钴26例(59.09%),硫酸镍15例(34.09%);重铬酸钾13例(29.55%);其次是芳香混合物11例(25.00%)。化妆品相关变应原阳性率较高的主要是乳化剂,如椰油酰胺丙基甜菜碱17例(45.95%),山梨糖醇酐倍半油酸酯16例(43.24%),过硫酸铵13例(35.14%),叔丁基氢醌12例(32.43%)。结论:面部不明原因皮炎患者欧洲标准变应原的斑贴试验结果与以往文献报道类似,反映该类群体普遍存在体质敏感的现象;而化妆品相关变应原的斑贴试验结果显示,此类患者主要对乳化剂等普通原料过敏,这与以往的文献报道有所不同。Objective： To analyze the allergic substances in unconfirmed facial dermatitis patients. Methods： Forty-four patients of with unknown cause facial dermatitis, who came from clinic service of the Department of Dermatology in Air Force General Hospital, investigated by patch tests with European standard combined with cosmetic related allergens, which was followed the recommended standard of the International Contact Dermatitis Research Group. Results： The higher positivity rate parts of the European standard al- lergens all belonged to the metal categories： cobalt chloride was 11 （59.09%）,nickel sulfate was 15 （34.09%）and potassium dichromate was 13 （29.55%）. Then the fragrance mix was 11 （25.00%）, The main cosmetic related allergens were emulsifier. For instance, cocami- dopropyl betaine was 17 （45.95%）, sorbitan monooleate was 16 （43.24%）, tert-butylhy droquinone was 13 （35.14%）, ammonium per- sulfate was 12 （32.43%）. Conclusions：the results of patch tests in unconfirmed facial dermatitis patients were similar to previous reports, which indicated that such group was generally in sensitive physical conditions. However the results of patch tests in cosmetic related allergens indicated that such group was mainly allergenic to ordinary material such as emulsifying agent, which was different from previous reports.
