电针对活动性类风湿关节炎患者关节疼痛与生存质量的影响

摘　　要：目的观察电针对活动性类风湿关节炎患者的疗效及对其生存质量的影响。方法将63例患者随机分为电针组(32例)与对照组(31例),两组均采取整体与局部相结合的配穴方法针刺治疗,电针组在此基础上加用电针治疗。评价两组患者疗效(包括ACR-20、ACR-50标准),并观察两组患者治疗前后临床症状、病情活动度、健康状况、生存质量(包括总体健康、躯体功能、生理职能、躯体疼痛、活力、社会功能、情感职能、精神健康)及炎性指标(包括血沉、C反应蛋白)的变化。结果治疗后电针组达到ACR-20和ACR-50标准的患者分别为22例(68.75%)和6例(18.75%);对照组分别为13例(41.94%)和4例(12.90%),两组患者达到ACR-50的例数比较无统计学意义(P>0.05),达到ACR-20的例数比较有统计学意义(P<0.05)。两组患者治疗后临床症状、病情活动度、健康状况均较治疗前明显改善(P<0.05或P<0.01),且电针组改善程度优于对照组(P<0.05)。生存质量方面,治疗后电针组除情感职能外,其余维度均较治疗前有明显提高(P<0.05或P<0.01);对照组总体健康、躯体功能、躯体疼痛、活力、社会功能均较治疗前有提高(P<0.05)。两组治疗后比较,在提高躯体功能、生理职能、躯体疼痛、精神健康方面电针组优于对照组(P<0.05)。两组患者治疗后血沉和C反应蛋白均较本组治疗前明显改善(P<0.01)。结论电针能有效缓解活动性类风湿关节炎患者关节的疼痛并提高患者的生存质量,其治疗总体效果优于单纯针刺法。Objective To observe the influence of electroacupuncture on active rheumatoid arthritis（RA）patients and their quality of life.Methods The 63 patients were randomized into an electroacupuncture group（32 cases）and a control group（31 cases）.Both groups were treated with whole body and local point selection method.Electroacpuncture was additionally used in the treatment group.The effect was evaluated with ACR-20 and ACR-50 standards and changes of clinical symptoms,activity of pathological condition,health status,quality of life（including overall health,somatic function,physical functions,somatic pain,vitality,social functions,emotional functions,mental health）and inflammatory indexes（including ESR and C-reactive protein）before and after treatment were observed.Results After treatment,there were 22 cases（68.75%）reached ACR-20 standard and 6 cases（18.75%）reached ACR-50 standard in the electroacupuncture group,and 13 cases（41.94%）reached ACR-20 standard and 4 cases（12.90%）reached ACR-50 standard in the control group.There was no significant difference in number of cases to ACR-50 standard（P0.05）,but there was a significant difference to ACR-20（P0.05）.An obvious improvement of clinical symptoms,activity of pathological condition,and health status after treatment existed in both groups（P0.05 or P0.01）and the electroacupuncture group was better than the control group.All indices except emotional functions in the electroaccupuncture group were significantly improved（P0.05 or P0.01）,while in the control group an improvement showed in overall health,somatic function,somatic pain,vitality,and social functions（P0.05）.In comparison,the electroacupuncture group was better in improving the somatic function,physical functions,somatic pain,and mental health than the control group（P0.05）.Both groups had a significant improvement on ESR and C-reactive protein（P0.01）after treatment.Conclusion Electroacupuncture can effectively relieve the pain of rheumatoid arthritis and
