摘 要:目的利用二尖瓣环自动追踪技术(AMAT)检测法洛四联症(TOF)患儿左心室容积、整体收缩功能及心尖摆动角度的变化。方法 TOF患儿30例,正常小儿30名,取心尖四腔心切面,运用AMAT技术自收缩期前(QRS波起点)测量左心室舒张末期容积、收缩末期容积、射血分数、面积、面积速率、容积速率、长轴长度、心尖摆动角度,并对以上指标进行统计学分析。结果与正常对照组对比,TOF患儿左心室射血分数、面积速率、容积速率差异无统计学意义(P>0.05);左心室舒张末期容积、收缩末期容积、面积、长轴长度与肺动脉环径/主动脉环径值(PA/AO)呈显著正相关;心尖摆动角度最大值、最小值均小于正常小儿(P<0.05)。结论 TOF患儿心尖摆动角度出现异常。AMAT技术可以评价TOF患儿左心室容积及收缩功能的变化。Objective To evaluate the change of left ventricular (LV) volume,total systolic function and apex movement in children with tetralogy of Fallot (TOF) with automated mitral annular tracking (AMAT). Methods LV end diastolic volume,end systolic volume,ejection fraction,LV area,area rate,volume rate,LV length,apex movement angle of the children with 30 TOF children and 30 normal children were detected with AMAT starting from pre-contraction in the apical four-chamber view. The indexes mentioned above were analyzed statistically. Results The LV ejection fraction,area rate,volume rate between the children with TOF and the normal children were not obviously different (P0.05). The end diastolic volume,end systolic volume,area,LV length were obviously positively correlated with the ratio of the diameter of pulmonary valvular annular to the diameter of aortic valvular annular (PA/AO). The apex-max angle and the apex-min angle of the children with TOF diminished compared with those of normal children (P0.05). Conclusion The apex movement of the children with TOF is different from the normal children. AMAT can be used to evaluate the changes of LV volume and total systolic function in children with TOF.
