达那唑和GnRH-a对ICPP女孩成年身高的影响

摘　　要：目的：研究达那唑和促性腺激素释放激素类似物（GnRH-a）对特发性中枢性性早熟（ICPP）女孩的治疗作用。方法：对不同处理的3组ICPP女孩（达那唑治疗组19例、GnRH-a治疗组14例和未治疗组19例）的成年身高（AHt）或预测成年身高（PHt）和遗传靶身高（THt）进行比较。结果：未治疗组42％达THt：达那唑治疗组84％达THt，且AHt高于未治疗组；GnRH-a治疗组治疗后PHt明显增加；达那唑通过促进生长，增加身高年龄增长与骨龄增长的比值（ΔHA／ΔBA）来改善成年身高；GnRH-a通过延缓骨骼成熟，减少骨龄增长与生活年龄增长的比值（BA／ΔCA）从而增加ΔHA／ΔBA来改善PHt。结论：达那唑和GnRH-a能有效地改善ICPP女孩的AHt和PHt，其序贯疗法尚需进一步研究。Objective: To evaluate the therapeutic effects of danazol and gonadotropin-releasing hormoneanalogue (GnRH-a) on the adult height (AHt) and predictive adult height (PHt) achieved in girls with idiopathic centralprecocious puberty (ICPP). Methods: To compare AHt and PHt fo the three following groups of girls with ICPP withtheir respective target height: 19 danazol treated, 14 GnRH-a treated and 19 untreated groups. Results: 42 % (8/19) of the AHt of the untreated girls and 84% (16/19) of the AHt of the danazol treated girls achieved their THt. ThePHt of GnRH-a treated girls increased significantly at the end of the treatment. Danazol improved the AHt byaccelerating growth and increasing the value of ΔHA/ΔBA, while GnRH-a exerted its effect on PHt by inhibiting theadvance of bone age (BA), decreasing the value of ΔBA/ΔCA and increasing ΔHA/ΔBA ratio. Conclusion: Danazoland GnRH-a can improve the AHt and PHt of girls with ICPP effectivelic The Consecutive treatment with danazol andGnRH-a needs to be further studied.
