摘  要:目的探讨螺旋CT脑灌注成像技术在超早期脑梗死诊断中的价值。方法分析总结12例发病在6h内临床可疑脑梗死,对其进行了CT平扫、CT脑灌注检查和CT复查住院患者资料,并对灌注资料做统计学分析。结果 CT平扫2例脑实质密度轻微减低、脑沟变浅(17%),CT脑灌注10例显示与临床症状对应区域血流灌注异常(83%),与对侧比较CBF、CBV下降,MTT延长。24~72h CT复查血流灌注异常区域全部出现大小不等低密度脑梗塞灶。2例对于CT平扫、CT灌注及复查均未见异常(17%)。结论常规CT平扫对超早期脑梗死诊断有限,不能满足临床的要求。CT脑灌注成像技术对超早期脑梗死诊断有重要的临床应用价值。Objective To investigate the application of spiral CT cerebral perfusion imaging in the diagnosis of acute infarction. Methods The data were analyzed of 12 patients,who fell ill in 6 hours,suspected of having acute ischemic infarction,were examined by non-enhanced computed tomography(CT),CT perfusion and examination by non-enhanced CT. These data were analyzed statistically. Results Non-enhanced CT scanning showed a slight reduction in the density of brain parenchyma in two cases,with brain shallow groove (17%)and CT perfusion showed perfusion abnormalities corresponding to clinical symptom region (83%)in 10 cases. Compared with the contra-lateral,CBF and CBV were decreased and MIT was extended. Two cases showed no abnormal characteristics by CT scanning, CT perfusion and CT review. Conclusion Non-enhanced CT is limited in the diagnosis of acute infarction and cannot meet the needs of clinical diagnosis and treatment. The clinical application of CT cerebral perfusion is of great importance to the diagnosis of acute infarction.
