益气通络法治疗足踝部皮肤软组织缺损60例临床观察

摘　　要：目的:观察益气通络法治疗足踝部皮肤软组织缺损的临床疗效。方法:将180例足踝部皮肤软组织缺损患者随机分为3组各60例。益气通络组应用益气通络法,口服活血通络丸治疗;活血化瘀组治以活血化瘀法,口服桃红四物汤;健脾益气组治以健脾益气法,口服四君子汤。3组均应用生肌玉红膏换药。治疗2周后,观察肉芽长出、肉芽与皮面齐平的时间及临床疗效。结果:治疗2周后,总有效率益气通络组100%,活血化瘀组75.00%,健脾益气组78.33%。益气通络组总有效率与活血化瘀组、健脾益气组比较,差异均有显著性意义(P<0.05)。肉芽长出、肉芽与皮面齐平时间益气通络组与活血化瘀组、健脾益气组比较,差异均有显著性意义(P<0.05)。活血化瘀组与健脾益气组比较,差异无显著性意义(P>0.05)。结论:益气通络法治疗足踝部皮肤软组织缺损能使创面的肉芽组织生长迅速,愈合时间明显缩短。Objective：To observe the effect of therapy of replenishing qi to unblock collaterals on skin and soft tissue defects of the dorsal foot and ankle. Methods： One hundred and eighty cases of skin and soft tissue defects of the dorsal foot and ankle were randomized into three groups （60 cases in each group）. Yiqi Tongluo group was treated with the decoction of Chinese medicine with the actions of replenishing qi to unblock collaterals, and with Huoxue Tongluo Pill. Huoxue Huayu group was treated with the decoction of Chinese medicine with the actions of activating blood and dissolving stasis, and with Tao Hong Siwu Decoction. Jianpi Yiqi group was treated with the decoction of Chinese medicine with the actions of tonifying spleen and qi, and with Sijunzi Decoction. All of the three groups were given external application of Shengji Yuhong Ointment. Two weeks later, the time for the early growth of granulation tissue and time for granulation tissue growing to join the skin were observed, and the clinic effect was evaluated. Results： After treatment, the total effective rate was 100% in Yiqi Tongluo group, 75.00% in Huoxue Huayu group, and 78.33% in Jianpi Yiqi group. The total effective rate in Yiqi Tongluo group differed from that in Huoxue Huayu group and Jianpi Yiqi group （P 〈 0.05）. Time for the early growth of granulation tissue and time for granulation tissue growing to join the skin in Yiqi Tongluo group differed from those in Huoxue Huayu group and Jianpi Yiqi group （P 〈 0.05）. There was no significant difference between Huoxue Huayu group and Jianpi Yiqi group （P 〉 0.05）. Conclusion： Therapy of replenishing qi to unblock collaterals is effective for skin and soft tissue defects of the dorsal foot and ankle through promoting the growth of the granulation tissue and shortening the healing time significantly.
