摘 要:[目的]探讨早期干预对早产儿体格智能发育的影响。[方法]将在四川省妇幼保健院住院分娩或就诊的早产儿146例,分为早期干预组71例和对照组75例,同时选择同期出生的足月儿为正常对照组,除外先天畸形和遗传代谢性疾病。[结果]干预组和对照组资料在胎龄、出生体重、头围差异无统计学意义(P﹥0.05)。早期干预组给予抚触、听、视、触觉刺激及按摩和婴儿操综合疗法,同时对家长进行健康教育,同时补充铁剂。对照组和正常对照组只接受常规保健指导。3组均于出生后定期接受体格发育监测、血常规检查和智力(CDCC)检查。[结果]干预组身高、体重、头围和智能发育指数(MDI)和运动发育指数(PDI)显著高于对照组(P﹤0.05),干预组贫血和营养不良的发生率显著低于对照组。而且干预组在12个月和18个月与正常对照组差异无统计学意义(P﹥0.05)。[结论]早期干预能促进早产儿智能和体格发育,并能减少贫血和营养不良的发生。[ Objective ] To explore the influence of early intervention on the physical and intellectual development of preterm infants. [ Methods] 146 preterm infants who delivered or visited in our hospital were divided into early intervention group (n = 71 ) and control group (n = 75), at same time. 80 term infants were selected as normal control group with no congenital deformity and hereditary metabolic diseases; no significant difference existed between the intervention group and the control group regarding with the birth weight, body height, head circunfference and gestational age (P 〉 0.05 ). The intervention group was given touching, massage, vision, hearing, vestibular stimuli and infant exercise after discharging from hospital.At same time, the infants were supplemented with iron and their parents were given health education.The infants of normal control group and control group received only conventional baby care. All the groups regularly had blood routine examination, physical development monitoring and CDCC. [ Results ] The mental developmental index (MDI). physical developmental index (PDI), weight, body height and head eireunfference of the intervention group were higher than that of the control group with a signifi- cant statistical significance (P 〈 0.05). The incidences of anemia and malnutrition of the intervention group were lower than that of the control group with a significant statistical significance No significant difference existed between the intervention group and normal control group in 12 and 18 months (P 〉 0.05). [Conclusion] The physical and intellectual development of preterm infants can be enhanced bv the practice of early intervention, and early intervention can reduce the incidences of anemia and malnutrition.
