摘 要:[目的]探讨脑胶质瘤CT灌注参数与MVD的相关性,为脑胶质瘤术前的诊断、分级以及判断预后提供一种安全可靠的方法。[方法]选择被CT或MR初步诊断为脑胶质瘤并且未经任何治疗的住院患者20例,应用CT灌注技术进行灌注扫描,测定感兴趣区和相应正常区域CBF、CBV、PS值并得到灌注图像。手术切除肿瘤,对相应感兴趣区进行HE染色明确病理类型,并采用免疫组织化学SP法染色测定MVD。采用SPSS14.0统计软件包分析数据,对对照组、低级别组、高级别组的CT灌注各参数值进行单因素方差分析和SNK检验,同时作低级别组与高级别组的CBV、CBF、PS与肿瘤MVD的pearson相关性分析。[结果](1)对照组、低级别组、高级别组的CBF分别为22.11±3.23;0.72±0.10;0.81±0.07,CBV分别为50.28±10.90;2.26±0.49;4.99±1.01,PS分别107.95±15.87;6.35±1.05;11.28±1.76,单因素方差分析F值分别为199.6;170.16;209.46,P值分别为0.00;0.00;0.00,差异有统计学意义。SNK检验进行组间比较,P值均小于0.01,差异有统计学意义。(2)低级别胶质瘤MVD计数为31.00±8.47;高级别胶质瘤MVD计数为60.60±7.96。(3)高、低级别组CBF、CBV、PS与MVD的统计学处理呈正相关,相关系数r值分别为0.79;0.85;0.77。[结论]CT灌注成像技术能准确反映肿瘤组织的灌注状态,评估肿瘤血管的增殖程度,可以为脑胶质瘤术前的诊断、分级以及判断预后提供一种安全可靠的方法。[Objective] To discuss the relationship between the CT perfusion parameters and MVD for a safe and reliable preoperative diagnosis to grade and prognosticate the brain glioma. [ Methods] Twenty hospitahzed untreated glioma patients primarily diagnosed by CT or MR were selected. CT Peffusion technique was performed. CBF, CBV, PS value were measured in the interested areas and corresponding normal areas and the perfusion images were obtained.Mter surgical resection of the tumor, HE stain was carried out in order to confirm pathological type of interested areas. MVD was detected by immunohistochemical SP staining. SPSS14.0 was used for statistical mlalysis. One-way ANOVA and SNK-test were applied for perfusion parameters of all groups. Pearson corelation analysis were carried out between perfusion parameters and MVD of the low grade group and the high grade group. [ Results] (1) The CBF values of the control group, low grade group and high grade group were 22.11 ± 3.23, 0.72±0.10, 0.81±0.07 respectively. The CBV values were 50.28±10.90, 2.26±0.49, 4.99±1.01 respectively and the PS values were 107.95±15.87, 6.35±1.05, 11.28±1.76 respeetively.F values were 199.6, 170.16, 209.46 respectively. P values were 0.00, 0.00, 0.00 respectively. P value was less than 0.01 by SNK-test among each group. (2) Low and high grade glioma MVD were 31.00±8.47, 60.60±7.96 respectively. (3) CBF, CBV, PS value of low and high grade glioma and tumor MVD presented positive correlation with statistical processing. Correlation coefficient value were 0.79, 0.85 and 0.77 respectively and P value was less than 0.01. [Conclusion] Rapid and noninvasive CT perfusion imaging technique can accurately reflect the peffusion status of the tumor tissue and estimate proliferation state of tumor vessels. It provides a safe and reliable method for preoperative diagnosis, grading and prognosis judgment of brain glioma.
