摘 要:目的制订早期活动方案,探讨其对预防脑出血患者术后深静脉血栓形成(deep venous thrombosis,DVT)的临床效果。方法按随机数字表法将100例脑出血术后患者分成试验组和对照组各50例,试验组除给予脑出血术后常规护理外,实施系统早期活动方案,即术后进行足踝被动或主动运动,腓肠肌、股二头肌、股四头肌和臀大肌等长收缩运动和深呼吸训练;对照组给予脑出血术后常规护理。分别在干预前及干预后第7日测量两组患者股静脉血流速度及DVT的发生情况。结果试验组股静脉血流速度明显高于对照组,而术后DVT发生率明显低于对照组。结论系统早期活动方案能提高下肢静脉血流速度,安全、有效地预防脑出血术后患者下肢DVT的形成。Objective To evaluate the effect of early exercise therapy on the formation of deep-venous thrombosis in post- operative patients with cerebral hemorrhage. Methods One hundred postoperative patients with cerebral hemorrhage were di- vided into experimental and control groups according to random number table. The patients in the control group received rou- tine nursing care. In addition,the patients in the experimental group received systematic early exercise therapy,including pas- sive or active ankle movement,isometric exercise of gastrocnemius,biceps femoris,quadrieeps and gluteus,and deep breathing training. The blood flow velocity in femoral vein and incidence of deep vein thrombosis were compared between the two groups before and at the seventh day after the intervention. Results The venous blood flow velocity in the experimental group was significantly higher,while the incidence of deep venous thrombosis was significantly lower than that in the control group after the intervention. Conclusion Systematic exercise therapy can increase the venous blood flow velocity,and thereby to prevent from forming DVT in patients with cerebral hemorrhage safely and effectively.
