摘 要:目的:评价磁共振水成像(MRH)技术对脑池段后组颅神经(cranial nerves,CN)及其毗邻血管解剖关系的显示效果和诊断价值。方法:检查对象共30例(60侧),其中男女各15例,平均年龄47岁。采用3.0T超导磁共振扫描仪,行横断面及平行于后组颅神经的斜矢状面水成像序列检查。结果:脑池段舌咽-迷走-副神经复合体在横断面和斜矢状面图像上的显示率分别为100%和95%;舌下神经在横断面和斜矢状面图像上的显示率分别为98.3%和91.7%。双侧舌咽-迷走-副神经复合体之间的平均距离女性为(2.06±0.12)cm,男性为(2.10±0.11)cm,其差异无统计学意义(P=0.32)。舌咽-迷走-副神经复合体长径与脑干正中矢状面的夹角左侧为54.42°±4.93°,右侧为56.97°±6.3°,统计学上差异有显著性意义(P=0.037),而不同性别之间差异无统计学意义。双侧舌咽-迷走-副神经复合体脑池段长度右侧(1.52±0.16)cm,左侧(1.57±0.19)cm;女性(1.51±0.14)cm,男性(1.54±0.17)cm,其在性别和侧别上的差异均无统计学意义(P=0.339和P=0.53)。与脑池段舌咽-迷走-副神经复合体有密切关系的血管主要是小脑后下动脉和椎动脉,在与神经有接触的该两类血管中,小脑后下动脉占70.8%。结论:MRH可以清晰显示后组颅神经以及与相邻脑血管的复杂解剖关系,发现该区域神经、血管性微小病变,为显微神经外科治疗计划的制定提供准确的影像信息。Objective:To evaluate the displaying efficiency and diagnostic value of magnetic resonance hydrogrophy sequence(MRH) for the cisternal segments of posterior cranial nerves and vessels adjacent to them.Methods:30 subjects(60 sides),including 15 men and 15 women,with mean age of 47 years,underwent FIESTA sequence on GE 3.0 magnetic resonance scanner,including axial and oblique sagittal imaging parallel to cranial nerves(CN) Ⅸ~Ⅺ.Results:On axial and oblique sagittal images of MRH,displaying rates of CN Ⅸ~Ⅺ complex were 100% and 95%,and those of CN Ⅻ were 98.3% and 91.7%,respectively.The mean distances of bilateral CN Ⅸ~Ⅺ were(2.06±0.12)cm in female and(2.10±0.11)cm in male,and the difference was not statistically significant(P=0.32).The angle between CN Ⅸ~Ⅺ and sagittal side of the brain stem were 54.42°±4.93° in the left side and 56.97°±6.33° in the right side,and the difference was significant statistically(P=0.037).The lengths of CN Ⅸ~Ⅺ complex were(1.52±0.16)cm in the right side,(1.57±0.19)cm in the left side,and(1.51±0.14)cm in female and(1.54±0.17)cm in male.There were no statistical significances in side and sex(P=0.339,0.53).In our series,the blood vessels close to CN Ⅸ~Ⅺ complex were posterior inferior cerebellar arteries(PICA) and vertebral arteries(VA).In the blood vessels contacting with CN Ⅸ~Ⅺ complex,PICAs were more often identified vessels and had a rate of 70.8%.Conclusion:In virtue of predominance on displaying cisternal segments of posterior cranial nerves and blood vessls close to them excellently,MRH can explore complex anatomic relationship between nerves and vessels in this region and find out micro lesions from them,so it provides very useful MR imaging sequences for the therapeutic program of microneurosurgery.
