摘 要:目的比较2008年和2009年北京某单位干部年度体检代谢综合征(MS)的检出率及影响因素。方法使用预定的流行病学调查表,由专业人员询问被检人群的既往史,测量身高、体重、血压,并计算体重指数(BMI),除确诊2型糖尿病者均统一行口服75g葡萄糖耐量试验(OGTT),测空腹和负荷后2h血糖、空腹血脂、血尿酸、肝肾功能等指标。分别计算2年的MS检出率。结果 2008年MS检出率为22.34%,2009年为40.69%,增加18.35%。MS检出率随年龄和BMI的增长而增加。高血压、糖尿病和糖尿病前期、高甘油三酯和(或)低高密度脂蛋白胆固醇、BMI≥25者的检出率2008年分别为52.95%、31.88%、29.79%、48.59%,2009年分别为67.48%、51.50%、45.32%、65.03%;2009年较2008年增加的各项均有统计学差异(P<0.01),净增率分别为14.53%,19.62%,15.53%,16.44%。Logistic多元回归分析显示,在调整年龄、性别后,BMI和甘油三酯的增长是影响MS检出率的主要危险因素。结论某单位干部人群具有较高的MS及其相关疾病的检出率,并呈快速增长趋势,MS的各组分检出率均明显增加,高血压和肥胖影响了半数以上的人群,BMI的增长尤需关注。Objective To compare the detection rate of metabolic syndrome(MS)and its influence factors in an institution of Beijing in the year of 2008 and 2009. Methods All participants completed a uniform questionnaire. Standard 75g oral glucose tolerance tests were done in participants without known diabetes. Each one had a physical examination including measurement of body height,body weight,blood pressure,and calculated body mass index(BMI). The fasting blood sample was collected for every one to determine blood lipid,uric acid,renal and hepatic function. And then the detection rates of MS in both years of these participants were analyzed. Results The detection rate of MS based on CDS criteria was 22.34% in 2008 and 40.69% in 2009,which was increased 18.35%. The prevalence of hypertension,diabetes and prediabetes,hypertriglycemia or/and hypo- hyper density lipoproteinemia and BMI≥25 was increased 14.53%,19.62%,15.53% and 16.44% respectively,being 67.48%,51.50%,45.32% and 65.03% in 2009,compared to 52.95%,31.88%,29.79%,48.59% in 2008(P0.01)respectively. The increment of BMI and triglyceridemia were independence risk factors for MS by Logistic multiple regression analysis after adjusted the effects of age and sex. Conclusions The detection rate of MS and related diseases were high in these participants, and increasing tendency was gross. There was more than half of this crowd with hypertension and obesity. Increased BMI should pay more attention.
