住院精神病患者临床感染138株大肠埃希菌药敏分析

摘  要:目的分析精神专科医院大肠埃希菌临床感染的耐药状况以及超广谱β-内酰胺酶(ESBLs)的检出情况,为临床合理使用抗生素提供可靠的实验依据。方法收集本院2008年10月至2010年4月间各种临床标本中分离的大肠埃希菌138株,应用BIO-KONT20E临床微生物鉴定分析系统进行分析鉴定,用K-B法进行药敏实验,同时,运用美国临床和实验室标准化研究所推荐方法检测ESBLs酶。结果大肠埃希菌对亚胺培南、阿米卡星、哌拉西林/他唑巴坦、头孢他啶、氨曲南、左氧氟沙星、头孢吡肟、妥布霉素,耐药率分别为0、3.62%、13.04%、17.39%、31.16%、31.88%、31.88%、38.41%,对头孢噻肟、头孢唑啉、头孢呋辛、头孢噻吩、复方新诺明、四环素、哌拉西林、安苄西林的耐药率均>60%。结论临床分离大肠埃希菌耐药率和耐药性逐渐增强,临床应结合药敏结果合理使用抗生素,减少耐药菌株的扩散,产ESBLs酶大肠埃希菌的检出应引起临床高度重视。Objective To investigate the antibiotic resistance and extended spectrum β-lactamases (ESBLs) production of Escherichia coli (E. coli) isolates obtained from the Mental Health Center of Yunnan Province and provide credible antimicrobial susceptibility test results for clinical antibiotic use.Methods A total of 138 clinical isolates of E. coli were collected from Mental Health Center of Yunnan Province from October 2008 to April 2010. Bacteria were identified with BIO-KONT 20E system, The Drug resistance was identified with K-B method. ESBLs were detected according to CLSI recommendations.Results The E. coli isolates were sensitive to Imipenem, amikacin, piperacillin/tazobactam, ceftazidime, aztreonam, levofloxacin, cefepime, tobramycin and the antibiotic resistance rates were 0%, 3.62%, 13.04%, 17.39%, 31.16%, 31.88%, 31.88% and 38.41% respectively. The isolates were resistant to cefotaxime, cefazolin, cefuroxime,thiophene, cotrimoxazole, tetracycline, piperacillin, ampicillin and the antibiotic resistances rates were all above 60%.Conclusion The study indicates antibiotic resistant E. coli isolates are increasing. Clinicians should use antibiotics according to the results of drug susceptibility test and pay more attention to ESBLs.
