19例胃肠型白塞病误诊误治分析

摘　　要：目的探讨临床上容易误诊误治的胃肠型白塞病的临床特点,以提高临床诊治水平。方法对19例诊治过程中存在误诊误治的胃肠型白塞病病人的发病年龄、病程、临床症状、消化道系统受累病变情况、辅助检查以及手术治疗等情况进行全面的分析。结果误诊误治的胃肠型白塞病病人平均病程较长[(10.2±7.5)年],临床出现消化道外症状明显早于出现消化道症状,至确诊时症状出现时间分别为(10.5±9.2)年和(3.5±5.8)年,两者比较差异有统计学意义(P<0.01)。外院诊治过程中,误诊为急性阑尾炎7例,误诊为肠结核6例,误诊为食管炎2例,2例误诊为浅表性胃炎、胃溃疡,另2例误诊为肠道感染。结论胃肠型白塞病消化道临床表现复杂多变,消化道症状出现晚,容易误诊、误治,全面提高对胃肠型白塞病的认识至关重要。Objective To investigate the clinical features of easily misdiagnosed and mistreated digestive system involved Behcet disease in clinic work, then improve clinic level of diagnosis and treatment. Methods In 19 cases of misdiagnosed and mistreated digestive system involved Behcet disease, their age of onset, course of disease, clinical symptoms, involved digestive system, auxiliary examination and operation were analyzed. Results Average course of misdiagnosed and mistreated digestive system involved Behcet disease was very long[（ 10. 2 ± 7. 5） years], clinical extra-digestive system complaint was significantly earlier than digestive system complaint[（ 10.5 ± 9.2） years vs （3.5 ± 5.8） years, respectively, P〈0. 01]. During the course of diagnosis and treatment, 7 cases were misdiagnosed as acute appendicitis, 6 cases as intestinal tuberculosis, 2 cases as esophagitis, 2 cases as superficial gas- tritis and gastric ulcer,and 2 cases as intestinal infection. Conclusion Digestive tract complaints were multiple and occurred lately in digestive system involved Behcet disease that was easily misdiagnosed and mistreated.
