双器械技术在中低位直肠癌保肛术中的应用

摘　　要：目的评价吻合器、闭合器双器械技术在中低位直肠癌保肛手术中的应用价值。方法2006年1月至2009年12月间应用吻合器、闭合器双器械技术行直肠前切除保留肛门治疗62例中低位直肠癌。结果本组无手术死亡，无吻合口瘘、吻合口狭窄发生。术后随访8～60个月，局部复发4例，其中Dukes B期1例，Dukes C期3例，复发率为6．5％。术后有不同程度的排便功能不良现象发生。结论吻合器、闭合器双器械技术的应用提高了中低位直肠癌保肛手术成功率，降低了吻合口瘘、吻合口狭窄的发生率。Objective To evaluate the applied value of double stapling technique in the sphincter-preserving surgery in middle and low rectal cancer. Methods Double stapling technique was applied in the sphincter-preserving surgery for 62 cases of middle and low rectal cancer from Jan. 2006 to Dec. 2009. Results There was no operative deaths, anastomotic leakage or constriction. There were 4 cases （6. 5%） of local recurrence during the follow-up period of 8-60 months, including 1 case of Dukes B, and 3 cases of Dukes C. Postoperative defecation dysfunction to varying degrees occurred. Conclusion The application of double stapling technique improves the success rate of the sphincter-preserving surgery in middle and low rectal cancer and reduces the recurrence of anastomotic leakage and anastomotic constriction.
